上颈段手法复位联合颈椎牵引治疗颈源性枕大神经痛的疗效分析

摘　　要：目的观察上颈段手法复位联合颈椎牵引治疗颈源性枕大神经痛的治疗效果,探讨颈源性枕大神经痛的错位分型和精准化治疗。方法选取2019年5月至2021年5月本院收治颈源性枕大神经痛患者180例,随机分为上颈段手法复位联合颈椎牵引治疗组(A组60例)、上颈段手法复位组(B组60例)及颈椎牵引组(C组60例)。三组患者治疗前后分别进行VAS评分和临床疗效评估。结果VAS评分及临床疗效评估,A组较B、C组差异有统计学意义(P<0.05);A组总有效率明显高于B、C组(P<0.05),B组高于C组。结论针对颈源性枕大神经痛,上颈段手法复位联合颈椎牵引治疗疗效优于单纯手法复位和单纯颈椎牵引治疗,其错位分型切合临床,且定位和操作较精准、安全,便于掌握。Objective To observe effect of upper cervical segments manipulative reduction and traction for the treatment of cervical greater occipital neuralgia,discuss the asymmetric grouping and precise management of cervical greater occipital neuralgia.Methods 108 patients with cervical greater occipital neuralgia were randomly divided into three groups according to the visiting orders:60 patients in group A treated upper cervical segments and upper cervical segments manipulative reduction and traction,60 patients in group B treated upper cervical segments,60 patients in group C treated cervical traction.Before and after treatment,we were calculated the visual analog scores(VAS)and assessed clinic therapeutic efficacy in the three groups.Results According the evaluation,there was the significantly different in group A to group B and C(P<0.05),the total effective rate in group A was 96.7%,88.3%in group B,and 76.7%in group C.The total effective rate of group A was significantly higher than the other two groups(P<0.05).And the effective rate of group B was higher than group C.Conclusion The treatment of upper cervical segments manipulative reduction and traction for cervical greater occipital neuralgia are better simple manipulative reduction or cervical traction,which has better precision and safety when locating and practicing and is easy to train.The asymmetric grouping are more conform to clinic.
