输注深低温冻存自体造血干细胞不良事件发生现状及影响因素分析

摘　　要：目的探讨输注深低温冻存自体造血干细胞不良事件发生现状和相关影响因素。方法选取2019年9月至2022年9月本院血液移植中心行自体造血干细胞移植的患者224例,采用一般资料调查表及输注自体造血干细胞不良事件调查表收集资料,汇总输注深低温冻存自体干细胞不良事件发生现状并分析影响因素。结果输注深低温冻存自体造血干细胞不良事件的总发生率为72.3%。胃肠道疾病发生率最高,占33.3%,其次是呼吸道、胸腔和纵隔疾病,占30.9%。严重程度多为1级,占68%。引起不良事件发生的危险因素为年龄、输注量、既往输血史和合并其他疾病。结论输注深低温冻存自体造血干细胞的不良事件发生率高,但大部分程度轻微。临床医护人员在围输注期,对于高龄、输注量较大、有输血史和合并其他疾病的患者应注意输注速率、加强监护,提供预见性护理措施,以保障患者安全。Objective To explore the current situation and related factors of adverse events of transfusion of cryopreserved autologous hematopoietic stem cells,and provide reference for clinical practice.Methods Using the convenient sampling method,224 patients who underwent autologous hematopoietic stem cell transplantation in the blood transplantation center of a third-class hospital in Wenzhou City from September 2019 to September 2022 were selected as the study subjects.The data were collected using the general data questionnaire and the adverse events questionnaire of autologous hematopoietic stem cell infusion,and the current situation of adverse events of autologous stem cell infusion in deep hypothermia was summarized and the influencing factors were analyzed.Results The total incidence of adverse events of transfusion of cryopreserved autologous hematopoietic stem cells was 72.3%.The system organ with the highest incidence rate was gastrointestinal diseases,accounting for 33.3%,followed by respiratory,thoracic and mediastinal diseases,accounting for 30.9%.The severity is mostly level 1,accounting for 68%.The risk factors causing adverse events were age,transfusion volume,previous blood transfusion history and other diseases.Conclusion The incidence of adverse events of transfusion of cryopreserved autologous hematopoietic stem cells is high,but most of them are mild.During the peri-infusion period,clinical medical staff should pay attention to the infusion rate,strengthen monitoring and provide predictive nursing measures for the elderly patients with large infusion volume,history of blood transfusion and other diseases to ensure the safety of patients.
