化滞柔肝颗粒对湿热中阻型非酒精性脂肪性肝病患者的临床疗效及对炎症因子的影响

摘　　要：目的探讨化滞柔肝颗粒对湿热中阻型非酒精性脂肪性肝病(NAFLD)的临床治疗效果,并观察其对外周血清中炎症因子的干预作用。方法选取2021年1月至12月武汉市第三医院就诊的湿热中阻型NAFLD患者90例作为研究对象,采用随机数字表法将其分为治疗组和对照组,每组45例。对照组在基础治疗的基础上加用多烯磷脂酰胆碱胶囊,治疗组在基础治疗的基础上加用化滞柔肝颗粒,两组均连续治疗3个月。比较两组治疗前后的体重指数、血脂、肝功能及炎症指标(Toll样受体-4、髓样分化因子88、核因子κB)。结果治疗后,两组甘油三酯,丙氨酸转氨酶,天冬氨酸转氨酶水平均低于治疗前,且治疗组低于对照组(P<0.05)。治疗后,治疗组体重指数低于治疗前,且治疗组低于对照组(P<0.05)。治疗后,两组Toll样受体-4、髓样分化因子88、核因子κB、肿瘤坏死因子-α、白细胞介素-1β水平均低于治疗前,且治疗组低于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。两组治疗前后体温、心率等生命体征,及心电图等安全性指标未发现明显变化。结论化滞柔肝颗粒治疗湿热中阻型NAFLD临床效果显著,可减轻体重,下调体重指数,改善血脂与肝功能,下调炎症因子,抑制炎症反应。Objective To investigate the clinical effect of Huazhi Rougan Granules in treating nonalcoholic fatty liver disease(NAFLD)with damp-heat blocking middle jiao type and to observe the intervention of inflammatory factors in peripheral serum.Methods A total of 90 patients with damp-heat blocking middle jiao type NAFLD treated in Wuhan Third Hospital from January to December 2021 were selected as the study objects,and they were divided into the treatment group and the control group by random number table method,with 45 cases in each group.The control group was treated with Polyene Phosphatidylcholine Granules on the basis of basic treatment,and the treatment group was treated with Huazhi Rougan Granules on the basis of basic treatment.Both groups were treated continuously for three months.Body mass index,blood lipids,liver function,and inflammatory indexes(Toll-like receptor-4,myeloid differentiation factor 88,nuclear factor-κB)were compared between the two groups before and after treatment.Results After treatment,the levels of triglyceride,alanine aminotransferase,and aspartate aminotransferase in two groups were lower than before treatment,and those in the treatment group were lower than those in the control group(P<0.05).After treatment,the body mass index of the treatment group was lower than before treatment,and that in the treatment group was lower than that in the control group(P<0.05).After treatment,the levels of toll-like receptor-4,myeloid differentiation factor 88,nuclear factor-κB,tumor necrosis factor-α,and interleukin-1βin two groups were lower than before treatment,and those in the treatment group were lower than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).No significant changes were found in vital signs such as body temperature,heart rate,and electrocardiogram before and after treatment.Conclusion Huazhi Rougan Granules has significant clinical efficacy in the treatment of damp-heat blocking midd
