基于奥瑞姆自理模式的护理干预对急诊创伤骨折患者心理应激反应及疼痛情况的影响

摘　　要：目的 探讨基于奥瑞姆(Orem)自理模式的护理干预在急诊创伤骨折患者中的应用。方法 选取2022年1月至12月浙江省丽水市中心医院接受治疗的108例急诊创伤骨折患者为研究对象,按照随机数字表法将其分为A组和B组,每组54例。A组采用常规护理,B组在常规护理基础上,采用基于Orem自理模式的护理。两组术后均随访3个月。比较两组救治相关指标、心理应激反应[焦虑自评量表(SAS)及抑郁自评量表(SDS)]、疼痛情况[视觉模拟评分法(VAS)]、舒适状态[舒适状态量表(GCQ)]评分及并发症。结果 B组救治相关指标所需时间均短于A组(P<0.05)。干预后,两组SAS、SDS、VAS评分均低于干预前,GCQ评分高于干预前,且B组SAS、SDS、VAS评分低于A组,GCQ评分高于A组(P<0.05)。B组并发症总发生率低于A组(P<0.05)。结论 基于Orem自理模式的护理干预可促进急诊创伤骨折患者急救及康复效果,减轻患者心理应激反应及疼痛程度,进一步提高患者舒适度,并有效减少并发症发生。Objective To explore the application of nursing interventions based on Orem self-care model in patients with emergency trauma fracture.Methods A total of 108 patients with emergency traumatic fracture treated in Lishui Central Hospital of Zhejiang Province from January to December 2022 were selected as the study objects,and they were divided into group A and group B according to random number table method,with 54 cases in each group.Group A was treated with routine nursing,group B was treated with Orem self-care model on the basis of routine nursing.Both groups were followed up for three months.The indexes related to treatment,psychological stress response(self-rating anxiety scale[SAS]and self-rating depression scale[SDS]),pain(visual analogue scale[VAS]),comfort status(general comfort questionnaire[GCQ])score,and complications were compared between the two groups.Results The time required for treatment related indexes in group B were shorter than those in group A(P<0.05).After intervention,the scores of SAS,SDS,and VAS in both groups were lower than before intervention,and the scores of GCQ were higher than before intervention,and the scores of SAS,SDS,and VAS in group B were lower than those in group A,and the score of GCQ in group A was higher than that in group A(P<0.05).The total complication rate of group B was lower than that of group A(P<0.05).Conclusion Nursing interventions based on Orem self-care model could promote the first aid and rehabilitation effects of emergency trauma fracture patients,reduce their psychological stress reactions and pain levels,further improve their comfort,and effectively reduce the occurrence of complications.
