凉血消风汤联合米诺地尔治疗血热风燥型脂溢性脱发患者的临床观察

摘　　要：目的 探究凉血消风汤辅助米诺地尔治疗血热风燥型脂溢性脱发的临床疗效及安全性。方法 选取2020年10月—2022年10月期间张家港市第一人民医院收治的脂溢性脱发患者102例作为研究对象。依据简单随机数字表法分为对照组和研究组,每组各51例。对照组采取米诺地尔治疗,研究组在对照组基础上联合凉血消风汤治疗,均治疗6个月。观察比较两组患者治疗前后中医证候积分、皮肤病生活质量量表(Dermatology Life Quality Index,DLQI)评分、生化指标[血管内皮生长因子(Vascular endothelial growth factor,VEGF)、转化生长因子-β1(Transforming growth factor-β one,TGF-β1)、胰岛素样生长因子-1(Insulin like growth factor-1,IGF-1)]水平,临床疗效及不良反应发生情况。结果 治疗后研究组临床总有效率93.88%(46/49)明显高于对照组79.59%(39/49),差异有统计学意义(P<0.05)。治疗后两组患者白屑、头发干枯、头皮瘙痒、头发稀疏脱落评分均较治疗前降低,差异有统计学意义(P<0.05);且研究组白屑、头发干枯、头皮瘙痒、头发稀疏脱落评分均明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者DLQI评分均较治疗前降低,差异有统计学意义(P<0.05);且研究组DLQI评分明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清VEGF、IGF-1水平均较治疗前升高,TGF-β1均水平较治疗前降低,差异有统计学意义(P<0.05),且研究组血清VEGF、IGF-1水平均明显高于对照组,TGF-β1水平明显低于对照组,差异有统计学意义(P<0.05)。治疗期间,研究组不良反应发生率8.16%(4/49)与对照组4.08%(2/49)比较,差异无统计学意义(P>0.05)。结论 采用凉血消风汤联合米诺地尔治疗血热风燥型脂溢性脱发可有效缓解临床症状,调节VEGF、IGF-1、TGF-β1水平,提升整体治疗效果,有利于改善患者生活质量,且具有安全性。Objective To explore the clinical efficacy and safety of Liangxue Xiaofeng Decoction assisted Minoxidil in the treatment of seborrheic alopecia caused by blood hot and wind dryness.Methods A total of 102 patients with seborrheic alopecia in our hospital from October 2020 to October 2022 were selected and divided into a study group(n=51)and a control group(n=51)according to the simple random number table method.The control group was treated with Minoxidil,while the study group was treated with Minoxidil combined with Liangxue Xiaofeng Decoction for 6 months.TCM syndrome scores,clinical efficacy,skin disease Quality of Life Scale(DLQI)scores,levels of biochemical indexes[vascular endothelial growth factor(VEGF),transforming growth factor-β1(TGF-β1),insulin-like growth factor-1(IGF-1)]and the occurrence of adverse reactions in the two groups before and after treatment were analyzed.Results After treatment,the total effective rate of the study group(93.88%)was higher than that of the control group(79.59%)(P<0.05).The scores of white dust,dry hair,scalp pruritus,and sparse hair loss in the two groups were lower than before treatment(P<0.05),and the study group was lower than the control group(P<0.05).After treatment,the DLQI scores of the two groups were lower than before treatment(P<0.05),and the study group was lower than the control group(P<0.05).After treatment,the serum levels of VEGF and IGF-1 in the two groups were higher than before treatment,while the level of TGF-β1 was lower than before treatment(P<0.05),and the serum levels of VEGF and IGF-1 in the study group were higher than those in the control group,while the level of TGF-β1 was lower than those in the control group with statistically significant difference(P<0.05).There was no significant difference in the incidence of adverse reactions between the study group(8.16%)and the control group(4.08%)(P>0.05).Conclusion Liangxue Xiaofeng Decoction assisted Minoxidil in the treatment of seborrheic alopecia caused by blood hot wind dryness can effectively
