彩色多普勒超声对卵巢黄体破裂与宫外孕破裂的鉴别诊断价值

摘　　要：目的分析彩色多普勒超声对卵巢黄体破裂与宫外孕(EP)破裂的鉴别诊断价值。方法回顾性分析53例EP破裂患者(EP破裂组)与52例卵巢黄体破裂患者(卵巢黄体破裂组)的资料。两组患者均采用彩色多普勒超声检查,比较两组患者彩色多普勒超声检测指标、血流动力学参数与血流表现。结果EP破裂组彩色多普勒超声卵巢体积(5.38±1.50)cm^(3)、异常回音区直径(3.24±1.50)cm、液性暗区深度(5.07±1.82)cm均小于卵巢黄体破裂组的(7.62±2.08)cm^(3)、(7.42±1.86)cm、(6.40±1.76)cm(P<0.05)。EP破裂组彩色多普勒超声收缩期峰值血流速度(12.08±3.23)cm/s小于卵巢黄体破裂组的(15.56±4.08)cm/s,搏动指数(0.94±0.25)、阻力指数(0.56±0.10)大于卵巢黄体破裂组的(0.65±0.18)、(0.34±0.07)(P<0.05)。EP破裂组彩色多普勒超声血流表现为环状或半环状发生率16.98%低于卵巢黄体破裂组的73.08%,条状发生率32.08%与点状发生率50.94%高于卵巢黄体破裂组的11.54%、15.38%(P<0.05)。结论彩色多普勒超声对于EP破裂与卵巢黄体破裂具有显著的鉴别诊断价值,适于临床应用与推广。Objective To analyze the value of color Doppler ultrasound in the differential diagnosis of rupture of corpus luteum and ectopic pregnancy(EP)rupture.Methods The data of 53 patients with EP rupture(EP rupture group)and 52 patients with rupture of corpus luteum(rupture of corpus luteum group)were retrospectively analyzed.Both groups of patients were examined by a color Doppler ultrasound,and the ultrasound detection indicators,as well as hemodynamic parameters and blood flow performance of the two groups of patients were compared.Results In EP rupture group,the ovarian volume by color Doppler ultrasound was(5.38±1.50)cm^(3),the abnormal echo region diameter was(3.24±1.50)cm,and the depth of the liquid dark region was(5.07±1.82)cm,which were all smaller than(7.62±2.08)cm^(3),(7.42±1.86)cm and(6.40±1.76)cm in rupture of corpus luteum group(P<0.05).In EP rupture group,the peak systolic blood flow velocity by color Doppler ultrasound was(12.08±3.23)cm/s,which was lower than(15.56±4.08)cm/s in rupture of corpus luteum group;EP rupture group had pulsation index of(0.94±0.25)and resistance index of(0.56±0.10),which were greater than(0.65±0.18)and(0.34±0.07)in rupture of corpus luteum group(P<0.05).The incidence of annular or semicircular color Doppler flow imaging in the EP rupture group was 16.98%,which was lower than 73.08%in rupture of corpus luteum group;the incidence of strip and dot appearance were 32.08%and 50.94%,which were higher than 11.54%and 15.38%in rupture of corpus luteum group(P<0.05).Conclusion Color Doppler ultrasound has significant differential diagnostic value for rupture of EP and corpus luteum,and is suitable for clinical application and promotion.
