宣白承气汤加味结合机械通气治疗急性呼吸窘迫综合征疗效观察

摘　　要：目的探讨宣白承气汤加味结合机械通气治疗急性呼吸窘迫综合征的临床效果。方法选取深圳市中医院2018年6月—2022年6月收治的106例急性呼吸窘迫综合征患者,分为观察组、对照组,各53例。给予对照组机械通气治疗,在此基础上给予观察组宣白承气汤加味治疗。比较两组ICU住院时间与机械通气时间、中医证候积分、呼吸参数及血气指标、血清降钙素原(procalcitonin,PCT)、白细胞介素(interleukin,IL)-1β、丙二醛(malondialdehyde,MDA)、二胺氧化酶(diamine oxidase,DAO)水平及安全性。结果治疗后两组中医证候积分及呼吸指数(respiratory index,RI)、血清PCT、IL-1β、MDA、DAO均降低,观察组更低;观察组ICU住院时间与机械通气时间更短;两组肺动态顺应性(dynamic lung compliance,Cdyn)、血氧分压(partial blood oxygen pressure,PaO 2)均升高,观察组更高(P<0.05)。结论宣白承气汤加味结合机械通气治疗急性呼吸窘迫综合征可改善患者证候及血气指标,有助于减轻氧化应激及炎性损伤,安全性高。Objective To investigate the clinical effect of Xuanbai Chengqi Decoction(宣白承气汤)combined with mechanical ventilation in the treatment of acute respiratory distress syndrome.Methods A total of 106 patients with acute respiratory distress syndrome admitted to the hospital from June 2018 to June 2022 were selected and divided into the observation group and the control group,with 53 cases in each group.The control group was given routine mechanical ventilation treatment,and the observation group was combined with Xuanbai Chengqi Decoction on the basis of the the control group.ICU stay time and mechanical ventilation time,TCM syndrome scores,respiratory parameters and blood gas related indexes,serum levels of procalcitonin(PCT),interleukin-1β(IL-1β),malondialdehyde(MDA),diamine oxidase(DAO)and safety of the observation group and the control group were compared.Results After treatment,TCM syndrome scores,respiratory index(RI),partial pressure of carbon dioxide(PaCO 2),serum levels of PCT,IL-1β,MDA and DAO all of the two groups were decreased and the levels of the observation group were lower.The length of ICU stay and mechanical ventilation of the observation group were shorter.The values of dynamic lung compliance(Cdyn)and partial blood oxygen pressure(PaO 2)of the two groups increased,and the values of the observation group was higher(P<0.05).Conclusion Xuanbai Chengqi Decoction combined with mechanical ventilation can improve the syndrome and blood gas indexes of acute respiratory distress syndrome,and help to reduce oxidative stress and inflammatory damage of the body,with high overall safety.
