IVF助孕中男方精子不同DFI水平对胚胎发育和妊娠结局的影响

摘　　要：目的探讨体外受精(IVF)助孕中男方精子不同DNA碎片率(DFI)对胚胎发育和妊娠结局的影响。方法选取2020年7月至2022年1月于临汾市人民医院行IVF助孕且随访至2023年6月的80对不孕夫妇作为研究对象,根据男方精子DFI水平分为DFI<25%组(n=44)和DFI≥25%组(n=36)。对比两组囊胚形成率、可利用囊胚率、优势囊胚率、精子总数、精子浓度、前向运动精子百分率、前向运动精子浓度、前向运动精子总数、不动精子总数、囊胚种植率、临床妊娠率、流产率;采用Logistic回归模型分析DFI对胚胎发育和妊娠结局的影响。结果DFI<25%组的囊胚形成率、可利用囊胚率、优势囊胚率均高于DFI≥25%组(P<0.05)。DFI<25%组的精子总数、精子浓度、不动精子总数均低于DFI≥25%组,而前向运动精子百分率、前向运动精子浓度、前向运动精子总数高于DFI≥25%组(P<0.05)。DFI<25%组的囊胚种植率、临床妊娠率均高于DFI≥25%组(P<0.05);两组流产率比较,差异无统计学意义(P>0.05)。Logistic回归分析显示,DFI是囊胚形成率、可利用囊胚率、优势囊胚率、囊胚种植率、临床妊娠率的影响因素(P<0.05)。结论男方不同精子DFI水平会对行IVF助孕患者的胚胎发育和妊娠结局产生影响。Objective To investigate the effect of different DNA fragmentation index(DFI)of sperm in in vitro fertilization assisted pregnancy men on embryonic development and pregnancy outcome.Methods A total of 80 infertility couples who received in vitro fertilization(IVF)assisted pregnancy in Linfen People′s Hospital from July 2020 to January 2022 and followed up to June 2023 were selected as the study subjects.According to the male sperm DFI level,they were divided into DFI<25%group(n=44)and DFI≥25%group(n=36).Blastocyst formation rate,available blastocyst rate,dominant blastocyst rate,total sperm count,sperm concentration,forward motile sperm rate,forward motile sperm concentration,total forward motile sperm count,total immotile sperm count,blastocyst implantation rate,clinical pregnancy rate and abortion rate were compared between the two groups.Logistic regression model was used to analyze the effects of DFI on fetal development and pregnancy outcome.Results The blastocyst formation rate,available blastocyst rate and dominant blastocyst rate in DFI<25%group were higher than those in DFI≥25%group(P<0.05).The total sperm count,sperm concentration and total immotile sperm count in DFI<25%group were lower than those in DFI≥25%group,but the rate of forward motile sperm,concentration of forward motile sperm and total forward motile sperm count were higher than those in DFI≥25%group(P<0.05).The blastocyst implantation rate and clinical pregnancy rate in DFI<25%group were higher than those in DFI≥25%group(P<0.05).There was no significant difference in abortion rate between the two groups(P>0.05).Logistic regression analysis showed that DFI was the influencing factor of blastocyst formation rate,available blastocyst rate,dominant blastocyst rate,blastocyst implantation rate and clinical pregnancy rate(P<0.05).Conclusions Different DFI levels of male sperm can affect the embryo development and pregnancy outcome of IVF assisted pregnancy patients.
