摘　　要：宫颈癌是最常见的威胁女性健康的生殖道恶性肿瘤,高危型人乳头瘤病毒(HR-HPV)感染是发生癌前病变和浸润性宫颈癌的先决条件。近年来其发病率持续上升,且感染患者呈年轻化趋势,不仅威胁着患者的身体健康,而且对患者造成巨大心理负面影响。因此,提高宫颈癌的筛查率以便尽早发现并干预疾病的发展极为重要,HPV筛查也成为了识别宫颈癌前病变和浸润性宫颈癌的关键策略。当HPV感染发展为持续性时,HPV基因组整合到宿主细胞基因组,E6、E7过度表达,同时造成基因组不稳定,最终导致宫颈组织发生癌前病变甚至演变为宫颈癌。当前,Aptima HPV检测是对HR-HPV E6/E7 mRNA表达量的定量检测手段,可以有效评估E6/E7癌基因在子宫颈上皮的活跃程度。本文主要分析Aptima HPV检测在宫颈癌前病变初筛中的优势,并对其相关研究进行综述,以期为临床HPV检测技术的合理应用提供参考。Cervical cancer is the most common reproductive tract malignant tumor that threatens women′s health.High-risk human papilloma virus(HR-HPV)infection is a prerequisite for precancerous lesions and invasive cervical cancer.In recent years,its incidence continues to rise,and infected patients are getting younger and younger,which not only threatens the physical health of female patients,but also has a huge negative impact on the psychology of female patients.Therefore,it is very important to improve the screening rate of cervical cancer in order to detect and interfere with the development of the disease as early as possible.HPV screening has also become a key strategy to identify cervical precancerous lesions and invasive cervical cancer.When HPV infection becomes persistent,the HPV genome will be integrated into the host cell genome,resulting in overexpression of E6 and E7,leading to genomic instability as well,resulting in cervical precancerous lesions and even cervical cancer in cervical tissues.At present,Aptima HPV detection is a quantitative test for HR-HPV E6/E7 mRNA expression,which can effectively evaluate the degree of E6/E7 oncogene activity in cervical epithelium.This paper mainly analyzes the advantages of Aptima HPV detection in the primary screening of cervical precancerous lesions,and reviews its related researches,in order to provide reference for the rational application of clinical HPV detection technology.
