摘　　要：目的:探讨体外冲击波疗法联合银离子敷料在慢性下肢溃疡患者中的治疗效果及对创面微环境的影响。方法:选择2023年3月~2023年12月慢性下肢溃疡患者75例为对象,信封法分为三组各25例。对照组采用银离子敷料干预,冲击波组采用体外冲击波治疗,研究组采用体外冲击波联合银离子敷料治疗,三组均连续完成2周干预,比较三组有效率、创面微环境(创面分泌物pH)、换药创面温度、Bates-Jensen伤口评估与效果评价量表(BWAT)评分。结果:研究组治疗2周有效率为92.00%高于冲击波组72.00%和对照组68.00%(P<0.05);冲击波组和对照组换药创面pH值无统计差异(P>0.05);研究组第二次、第三次及第四次换药创面pH值低于冲击波组和对照组(P<0.05);研究组第二、三、四次换药创面温度和BWAT评分低于冲击波组和对照组(P<0.05)。结论:体外冲击波疗法联合银离子敷料用于慢性下肢溃疡患者中,能提高患者有效率,降低创面分泌物pH值、创面温度及BWAT评分,改善创面微环境。Objective:To explore the therapeutic effect of extracorporeal shock wave therapy combined with silver ion dressings in patients with chronic lower limb ulcers and its impact on the wound microenvironment.Methods:75 patients with chronic lower limb ulcers from March 2023 to December 2023 were selected as the study subjects,and were divided into three groups using envelope method,with 25 cases in each group.The control group was treated with silver ion dressings,the shock wave group was treated with extracorporeal shock wave therapy,and the study group was treated with extracorporeal shock wave combined with silver ion dressings.All three groups completed two consecutive weeks of intervention.The effective rate,wound microenvironment(pH of wound secretions),wound temperature during dressing change,and Bates Jensen wound evaluation and efficacy evaluation scale(BWAT)scores were compared among the three groups.Results:The effective rate of the study group after 2 weeks of treatment was 92.00%,which was higher than that of the shock wave group at 72.00%and the control group at 68.00%(P<0.05);There was no statistically significant difference in the pH value of wound dressing between the shock wave group and the control group(P>0.05);The pH value of the wound during the second,third,and fourth dressing changes in the study group was lower than that of the shock wave group and the control group(P<0.05);The study group had lower wound temperature and BWAT scores for the second,third,and fourth dressing changes compared to the shock wave group and control group(P<0.05).Conclusion:The combination of extracorporeal shock wave therapy and silver ion dressings can improve the effective rate of patients with chronic lower limb ulcers,reduce the pH value of wound secretions,wound temperature,and BWAT score,improve the wound microenvironment.
