摘 要:目的 探讨血清维生素A、D水平与新生儿感染的严重程度及预后之间的关系。方法 收集2020年4月至2022年4月苏州大学附属儿童医院新生儿科住院的82例新生儿感染性疾病患儿为观察组;根据新生儿危重病例评分将其分为极危重组(20例)、危重组(25例)、非危重组(37例);根据预后情况,将其分为死亡组(11例)和生存组(71例)。选取同期住院非感染新生儿80例作为对照组。比较观察组与对照组血清维生素A、D水平;比较新生儿感染不同疾病重症程度患儿血清维生素A、D水平;分析血清维生素A、D水平与新生儿感染疾病重症程度的相关性;分析新生儿感染患儿预后的影响因素;评估血清维生素A、D对新生感染患儿预后的预测价值。结果 观察组维生素A、D水平低于对照组(P<0.05)。极危重组、危重组血清维生素A、D水平低于非危重组,极危重组血清维生素A、D水平低于危重组(P<0.05)。血清维生素A、D水平与新生儿感染疾病危重程度呈负相关(r=-0.458、-0.489,P<0.01)。死亡组降钙素原(PCT)、C反应蛋白(CRP)水平高于生存组,维生素A、D水平低于生存组(P<0.05)。PCT、CRP、维生素A、维生素D是新生儿感染预后的影响因素(P<0.05)。维生素A、D联合预测新生感染预后的曲线下面积高于各项单独预测。结论 新生儿感染时血清维生素A、D水平与疾病严重及预后密切相关,并可以作为预测预后的有效指标。Objective To investigate the relationship between serum vitamin A and D levels and the severity and prognosis of neonatal infection.Methods From April 2020 to April 2022,82 children with neonatal infection hospitalized in Department of Neonatology,Children's Hospital of Soochow University were collected as observation group;they were divided into extremely critical group(20 cases),critical group(25 cases),and non-critical group(37 cases) according to neonatal critical illness;according to the prognosis,they were divided into death group(11 cases) and survival group(71 cases).A total of 80 hospitalized non-infected neonates with different were selected as control group.Serum vitamin A and D levels were compared between observation group and control group;serum vitamin A and D levels in children with neonatal infection neonates with different disease severity were compared.The correlation between serum vitamin A and D levels and different disease severity of children with neonatal infection disease was analyzed;the factors influencing prognosis of children with neonatal infection was analyzed;and the predictive value of serum vitamin A and D in prognosis of children with neonatal infection was evaluated.Results Serum vitamin A and D levels in observation group were lower than those in control group(P<0.05).Serum vitamin A and D levels in extremely critical group and critical group were lower than those in non-critical group,and serum vitamin A and D levels in extremely critical group were lower than those in critical group(P<0.05).Serum vitamin A and D levels were negatively correlated with different disease severity of children with neonatal infection disease(r=-0.458,-0.489,P <0.01).procalcitonin(PCT) and C-reactive protein(CRP) levels in death group were higher than those in survival group,while vitamin A and D levels were lower than those in survival group(P<0.05).PCT,CRP,vitamin A,and vitamin D were the influencing factors of prognosis of children with neonatal infection(P<0.05).The area under the curve of vit
