摘　　要：本文主要介绍孙占学教授从郁火论治痤疮经验。孙占学教授基于古籍“郁乃痤”相关论述,以及多年临床经验提出了痤疮的病机演变与“郁火”相关。五脏郁滞,脂液凝结,发为粉刺;五郁化火,肉腐脓生,形成丘疹、脓疱;火灼血络,络损不复,形成痤疮后遗症。治疗上,孙占学教授主张分期论脏辨治痤疮。粉刺期痤疮应以调理脏气、开腠理郁为主;丘疹、脓疱期痤疮根据“火郁发之”理论以内疏外透为法;痤疮后遗症期则以行气活血、敛疮生肌为主要治法。并附孙占学教授运用自拟解郁消痤汤治疗痤疮验案一则,以期为临床提供思路参考。This article mainly introduces Professor Sun Zhanxue's experience in treating acne from fire stagnation.Professor Sun Zhanxue proposed that the pathogenesis of acne is related to “fire stagnation”,based on the relevant discussion in the ancient book about “stagnation can cause acne”and many years of clinical experience.Five zang viscera stagnation,fat condensation,forming pimple.Five stagnation turns into fire,flesh rots,and pus grow,forming papules and pustules.Fire burns blood collaterals,the damage to the collaterals cannot be restored,forming acne sequelae.In treatment,Professor Sun Zhanxue advocates staging on the differentiation of viscera to treat acne.In the pimple stage of acne,the main treatment is to regulate viscera qi,open hair follicles and regulate stagnation.Acne in papule and pustular period was treated with internal dredging and external dispersing according to the theory of “fire stagnation requiring dissipation”.In the sequelae stage of acne,the main treatment is to promote qi and activate blood,astringe sores and promote muscle growth.Attached a test case of Professor Sun Zhanxue used self-designed Jieyu Xiaocuo Decoction to treat acne,in order to provide clinical ideas for reference.
