摘　　要：目的 评价红蓝光照射联合藻酸盐敷料对肛周脓肿术后创面的愈合作用。方法 选取2021年1月至2022年12月浙江大学医学院附属第二医院临平院区行一次性根治术的肛周脓肿患者106例,按照随机数字表法将其分为对照组和研究组,每组53例。对照组采用常规护理及健康宣教,术后创面使用藻酸盐敷料换药治疗;研究组在对照组基础上加用红蓝光照射创面治疗。比较两组术后第1、7、14天疼痛、创面分泌物渗出、肉芽组织生长、伤口愈合时间、焦虑自评量表(SAS)、舒适度评分等变化情况。结果 两组疼痛评分整体分析发现,时间和分组存在交互作用(P<0.05);单独效应检验结果显示,不同时间点组间、组内比较,差异有统计学意义(P<0.05)。两组创面渗液评分和肉芽组织生长评分整体分析发现,时间与分组无交互作用(P>0.05);单独效应检验结果显示,不同时间点组间、组内比较,差异有统计学意义(P<0.05)。与对照组比较,研究组患者创面愈合时间明显缩短(P<0.05)。两组治疗前后SAS评分和舒适状况评分比较,差异有统计学意义(P<0.05)。结论 红蓝光照射联合藻酸盐敷料能减少肛周脓肿术后分泌物渗出,加快创面肉芽生长,促进创面愈合,减少疼痛及焦虑情绪,提高患者舒适度和生活质量,值得临床推广应用。Objective To evaluate the efficacy of red and blue light irradiation and alginate dressing on wound healing after perianal abscess.Methods A total of 106 inpatients with disposable radical perianal abscess in Linping District of the Second Affiliated Hospital of Zhejiang University School of Medicine from January 2021 to December 2022 were selected and divided into control group and study group by random number table method,with 53 cases in each group.The control group received routine care and health education,and the wound was treated with alginate dressing after surgery.The study group was received red and blue light irradiation on the wound based on the control group.The scors of pain,wound secretion exudate,granulation tissue growth,wound healing time,self-rating anxiety scale(SAS),and comfort postoperative 1th,7th day and 14th day were compared between two groups.Results The overall analysis of pain score found an interaction between time and groups(P<0.05).Individual effect statistics showed statistically significant differences between and within the two groups at different time points(P<0.05).The overall analysis of wound infiltration score and granulation tissue growth score had no interaction between time and groups(P>0.05);individual effect statistics showed statistically significant differences between and within the two groups at different time points(P <0.05).Compared with control group,the wound healing time of the study group was significantly shortened(P<0.05).There were significant differences in SAS scores and comfort scores between and within the two groups before and after treatment(P<0.05).Conclusion The red and blue light irradiation and alginate dressing can reduce postoperative exudation with perianal abscess,accelerate wound granulation growth,promote wound healing,reduce pain and anxiety,improve comfort and the patients' life quality.It is worthy of clinical application.
