加味地黄汤辅助治疗肝肾阴虚型老年糖尿病视网膜病变的临床研究

摘　　要：目的 探讨加味地黄汤辅助治疗肝肾阴虚型老年糖尿病视网膜病变(DR)的临床效果。方法 选取2021年5月至2023年1月海南省中医院收治的肝肾阴虚型老年DR患者108例,根据随机数字表法将其分为对照组和观察组。对照组(54例)接受常规降糖和羟苯磺酸钙胶囊治疗,观察组(54例)在对照组基础上接受加味地黄汤辅助治疗,以4周为1个疗程,连续治疗3个疗程。比较两组临床疗效和治疗前后中医证候积分、视力水平、血糖指标[空腹血糖(FBG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)]、血管内皮生长因子(VEGF)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)],并比较两组治疗安全性。结果 观察组临床疗效优于对照组(P<0.05)。治疗后,两组中医证候积分、FBG、2 hPG、HbA1c、VEGF、TNF-α、IL-6、MDA水平均低于治疗前,视力、SOD水平高于治疗前,且观察组中医证候积分、FBG、2 hPG、HbA1c、VEGF、TNF-α、IL-6、MDA水平均低于对照组,视力、SOD水平高于对照组(P<0.05)。两组治疗期间均未出现明显的不良反应。结论 加味地黄汤辅助治疗肝肾阴虚型老年DR的效果确切,能显著改善症状,提高视力水平,且安全性高。Objective To explore the clinical efficacy of Jiawei Dihuang Decoction in the treatment of elderly diabetic retinopathy(DR) with syndrome of yin deficiency of liver and kidney.Methods A total of 108 elderly DR patients with syndrome of yin deficiency of liver and kidney treated in Hainan Traditional Chinese Medicine Hospital from May 2021 to January 2023 were selected and divided into control group and observation group according to random number table method.The control group(54 cases) received routine hypoglycemic and Calcium Isobenesulfonic Acid Capsules,and the observation group(54 cases) received Jiawei Dihuang Decoction on the basis of the control group,with four weeks as a course of treatment for three consecutive courses.The clinical efficacy was compared between the two groups,traditional Chinese medicine syndrome score,visual acuity,blood glucose index(fasting blood glucose [FBG],2 h postprandial blood glucose [2hPG],glycosylated hemoglobin [HbA1c]),vascular endothelial growth factor(VEGF),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),oxidative stress index(superoxide dismutase [SOD],malondialdehyde [MDA]) were compared between the two groups before and after treatment,the safety of treatment was compared between the two groups.Results The clinical effect of the observation group was better than that of the control group(P<0.05).After treatment,traditional Chinese medicine syndrome score,FBG,2hPG,HbA1c,VEGF,TNF-α,IL-6,and MDA levels in both groups were lower than those before treatment,while visual acuity and SOD levels were higher than those before treatment.The traditional Chinese medicine syndrome score,FBG,2hPG,HbA1c,VEGF,TNF-α,IL-6,and MDA levels in the observation group were lower than those in the control group,and the visual acuity and SOD levels were higher than those in the control group(P<0.05).There were no obvious adverse reactions during treatment in both groups.Conclusion Jiawei Dihuang Decoction is effective in the treatment of elderly DR with syndrome of yin deficiency of liver
