摘　　要：目的 探讨基于多学科协作团队(MDT)纵向列队研究管理干预对老年糖尿病胰岛素注射患者自我监管及安全性的影响。方法 选取江苏省中医院内分泌科2022年2月至2023年7月收治的老年糖尿病胰岛素注射患者82例为研究对象,根据随机数字表法将其分为两组,每组41例。对照组实施常规管理干预模式,观察组在对照组基础上实施基于MDT纵向列队研究管理干预模式,干预3个月。比较两组干预前后自我注射和自我监测恐惧量表、自我管理行为量表评分及并发症发生率。结果 干预后,两组自我注射和自我监测恐惧量表评分均低于干预前,且观察组低于对照组(P<0.05)。干预后,两组自我管理行为量表各维度评分均高于干预前,且观察组高于对照组(P<0.05)。观察组并发症总发生率低于对照组(P<0.05)。结论 基于MDT纵向列队研究管理干预模式有助于减轻老年糖尿病胰岛素注射患者自身恐惧、抵抗等负性心理状态,提高其自我管理行为能力,防止并发症发生。Objective To explore effect of management intervention based on multidisciplinary team(MDT) longitudinal cohort study on self-regulation and safety in elderly diabetic patients with insulin injection.Methods Eighty-two elderly diabetic patients with insulin injection admitted to Department of Endocrinology in Jiangsu Province Hospital of Chinese Medicine from February 2022 to July 2023 were selected as research objects,and they were divided into two groups according to random number table method,with 41 cases in each group.Control group implemented routine management intervention mode,while observation group implemented management intervention mode based on MDT longitudinal cohort study on the basis of control group,with intervention period of three months.Scores of self-injection and self-monitoring fear scale and self-management behavior scale before and after intervention,and incidence rate of complications between two groups were compared.Results After intervention,the scores of self-injection and self-monitoring fear scales of two groups were lower than before intervention,and those in observation group were lower than those in control group(P<0.05).After intervention,the scores of each dimension of self-management behavior scale in two groups were higher than before intervention,and those in observation group were higher than those in control group(P<0.05).Total incidence rate of complications in observation group was lower than that in control group(P<0.05).Conclusion Management intervention model based on MDT longitudinal formation is helpful to reduce the negative mental state such as fear and resistance of elderly diabetic patients with insulin injection,improve their ability to self-management behavior and prevent complications.
