摘  要:目的分析C反应蛋白/白蛋白(CRP/ALB)和可溶性白细胞介素-2受体(sIL-2R)水平与新生儿肺部感染病情程度及预后的相关性。方法选取2019年6月—2022年6月期间青岛市胶州中心医院收治的120例肺部感染的新生儿,所有患儿均进行CRP、ALB及sIL-2R水平检测,计算CRP/ALB。所有新生儿均取肺部分泌物进行病原菌培养,根据是否为细菌感染分为细菌感染组及非细菌感染组,根据病情严重程度分为轻度感染组和重度感染组,统计并比较细菌感染组及非细菌感染组、轻度感染组和重度感染组新生儿CRP/ALB和sIL-2R水平。根据新生儿治疗2周后的疗效,将患儿分为预后良好组和预后不良组,比较不同预后患儿CRP/ALB和sIL-2R水平及临床资料。采用Logistic回归分析明确肺部感染新生儿预后的影响因素,并采用受试者工作特征曲线(ROC)分析CRP/ALB和sIL-2R水平对肺部感染新生儿预后的预测价值。结果与非细菌感染组比较,细菌组感染组CRP/ALB、sIL-2R水平均升高(P<0.05);与非细菌感染组比较,细菌组感染组CRP/ALB、sIL-2R水平均升高(P<0.05);120例肺部感染新生儿中83例预后良好,37例预后不良,预后不良率为30.83%;与预后不良组比较,预后不良组CRP/ALB、sIL-2R水平均升高(P<0.05);预后不良组新生儿出生体重、新生儿Apgar评分≤7分占比、剖宫产占比、新生儿窘迫占比、非母乳喂养占比均高于预后良好组(P<0.05);Logistic回归模型分析结果显示,新生儿Apgar评分≤7分、≤7分、CRP/ALB及sIL-2R高水平是影响肺部感染新生儿预后不良的独立危险因素(P<0.05)。ROC分析结果显示:CRP/ALB、sIL-2R单独及联合预测肺部感染新生儿预后不良的AUC值(95%CI)分别为0.764(0.678~0.817)、0.710(0.620~0.789)、0.862(0.787~0.918),CRP/ALB联合sIL-2R预测效能高于单独检测(Z=1.953、2.213,P<0.05)。结论重度肺部感染新生儿CRP/ALB、sIL-2R水平异常升高,CRP/ALB和sIL-2R高水平是影响肺部�Objective To analyze the correlation between the levels of C-reactive protein/albumin(CRP/ALB)and soluble interleukin-2 receptor(slL-2R)and the severity and prognosis of neonatal pulmonary infection.Methods A to-tal of 120 neonates with pulmonary infection were selected from June 2019 to June 2022.CRP,ALB and sIL-2R levels were measured in all children,and CRP/ALB was calculated.All neonates were cultured with pathogenic bacteria from lung secretions and divided into bacterial infection group and non-bacterial infection group according to whether it was bacterial infection.According to the severity of the disease,they were divided into mild infection group and severe infec-tion group.The levels of CRP/ALB and sIL-2R in neonates of bacterial infection group and non-bacterial infection group,mild infection group and severe infection group were compared.According to the therapeutic effect after 2 weeks of neonatal treatment,the children were divided into good prognosis group and poor prognosis group.The levels of CRP/ALB and sIL-2R in children with different prognosis and clinical data were compared.Logistic regression analysis was used to determine the factors affecting the prognosis of neonates with pulmonary infection.Receiver operating char-acteristic curve(ROC)was used to analyze the predictive value of CRP/ALB and sIL-2R levels on the prognosis of neo-nates with pulmonary infection.Results Compared with non-bacterial infection group,CRP/ALB and sIL-2R levels in bacterial infection group were increased(P<O.05).Compared with non-bacterial infection group,CRP/ALB and sIL-2R levels in bacterial infection group were increased(P<0.05).Among 120 neonates with pulmonary infection,83 cases had good prognosis,37 cases had poor prognosis,and the poor prognosis rate was 30.83%.Compared with poor prog-nosis group,CRP/ALB and sIL-2R levels were increased in poor prognosis group(P<0.05).The proportion of birth weight,Apgar score<7,cesarean section,neonatal distress,non-breastfeeding and invasive operation in poor prognosis g
