独活寄生汤加味治疗类风湿关节炎临床研究

摘　　要：目的探究独活寄生汤加味治疗类风湿关节炎(rheumatoid arthritis,RA)的临床疗效。方法将126例RA患者随机分为对照组和试验组,每组63例,每组各退出3例,最终各纳入60例。对照组予以甲氨蝶呤治疗,试验组在对照组基础上加用独活寄生汤加味治疗,2组均治疗3个月。比较2组临床总有效率、疼痛评分、生活能力评分、中医证候积分以及炎症因子水平。结果治疗后,试验组总有效率为93.33%,高于对照组的80.00%(P<0.05)。2组疼痛视觉模拟量表(visual analogue scale,VAS)评分、健康评估问卷(health assessment questionnaire,HAQ)评分、中医证候积分均较治疗前降低(P均<0.05),且试验组上述指标均低于对照组(P均<0.05)。2组C反应蛋白(C-reactive protein,CRP)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)等炎症因子水平均较治疗前降低(P均<0.05),且试验组上述指标均低于对照组(P均<0.05)。结论应用独活寄生汤加味治疗RA可提高临床疗效,缓解疼痛症状,改善中医证候积分,提高日常生活能力,并降低炎症因子水平,值得推广。Objective To explore the clinical efficacy of modified Duhuo Jisheng decoction in the treatment of rheumatoid arthritis(RA).Methods A total of 126 patients with RA were randomly divided into the control group and the trial group,with 63 cases in each group.Three cases were withdrew in each group,and ultimately 60 cases were included each.The control group was treated with methotrexate,while the trial group was treated with modified Duhuo Jisheng decoction on the basis of the control group.Both groups were treated for three months.The total clinical effective rate,pain scores,life ability scores,traditional Chinese medicine syndrome scores and levels of inflammatory factors were compared in the two groups.Results After treatment,the total effective rate of the trial group was 93.33%,which was higher than the 80.00%of the control group(P<0.05).The visual analogue scale(VAS)scores of pain,health assessment questionnaire(HAQ)scores and traditional Chinese medicine syndrome scores in the two groups were all lower than those before treatment(all P<0.05),and the above indicators in the trial group were all lower than those in the control group(all P<0.05).The levels of inflammatory factors such as C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were all decreased compared to those before treatment(all P<0.05),and the above indicators in the trial group were all lower than the control group(all P<0.05).Conclusion The application of modified Duhuo Jisheng decoction in the treatment of RA could improve clinical efficacy,alleviate pain symptoms,improve traditional Chinese medicine syndrome scores,enhance daily living abilities,and reduce levels of inflammatory factors,which is worth promoting.
