三级中医医院综合医疗服务能力评价指标体系构建

摘　　要：目的:构建一套既能体现综合医疗服务能力,又具有中医药服务特色的中医医院综合医疗服务能力评价指标体系,为中医医院的服务能力评价提供工具与依据。方法:采用文献法和德尔菲法基于Donabedian模型构建中医医院综合医疗服务能力评价指标体系,并运用层次分析法确定各指标权重。结果:经过两轮专家咨询,确定了一级指标3项、二级指标14项和三级指标77项。一级指标分别为结构维度、过程维度和结果维度,权重分别为0.5224、0.3065和0.1711。二级指标权重最高为人员结构(0.3275),三级指标权重最高为高级卫技人员数量占比(0.0777)。结论:基础设施与人员配备等结构要素是医疗服务能力提升的基础,医疗服务过程的完善是服务能力提升的关键,而结果性指标是医疗服务能力水平提升的导向。研究所构建的中医医院综合医疗服务能力评价指标能够较为全面、客观地评价中医医院服务能力,是一套科学有效便捷的评价工具。Objective:To develop an evaluation indicator system for comprehensive medical service capabilities of traditional Chinese medicine hospitals that reflects both general medical service capabilities and the unique features of traditional Chinese medicine,providing a tool and basis for evaluating the service capabilities of traditional Chinese medicine hospitals.Method:The system was constructed using the literature review and Delphi methods based on the Donabedian model,with the Analytic Hierarchy Process(AHP)used to determine the weights of each indicator.Results:After two rounds of expert consultations,three primary indicators,fourteen secondary indicators,and seventy-seven tertiary indicators were determined.The primary indicators include structural,process,and outcome dimensions,with weights of 0.5224,0.3065,and 0.1711 respectively.The highest weighted secondary indicator is personnel structure(0.3275),and the highest weighted tertiary indicator is the proportion of senior healthcare personnel(0.0777).Conclusion:Infrastructure and staff allocation as structural elements are foundational for enhancing medical service capability,perfecting the medical service process is key to capability enhancement,and outcome indicators guide the improvement of service levels.The constructed evaluation indicator system can comprehensively and objectively assess the service capabilities of traditional Chinese medicine hospitals,serving as a scientifically valid and convenient evaluation tool.
