子宫动脉上行支结扎术在剖宫产产后出血防控中的应用效果研究

摘　　要：研究子宫动脉上行支结扎术在剖宫产产后出血防控治疗中的应用价值。选择六枝特区妇幼保健院2020年7月—2023年9月收治的28例行剖宫产术分娩且存在产后出血高危风险的患者为研究对象,并将其随机分为对照组、研究组两组,对照组提前应用卡前列素氨丁三醇注射液开展预防性干预;研究组应用卡前列素氨丁三醇注射液,再开展子宫动脉上行支结扎术治疗。结果表明,研究组患者产后出血发生率为4.76%,对照组患者产后出血发生率为42.85%(P<0.05);研究组患者术后并发症发生率为28.57%,对照组患者术后并发症发生率为35.71%(P>0.05);研究组患者手术时间、住院时间更短,术中出血量更少,Hb以及RBC水平更高,与对照组相比,差异有统计学意义(P<0.05);两组患者术后首次月经恢复时间以及产后恶露时间对比,无统计学差异(P>0.05)。研究发现,子宫动脉上行支结扎术应用于剖宫产产后出血的防控治疗中效果突出,不会影响患者的卵巢、再生育或月经功能,有推广应用价值。Study the application value of uterine artery ascending branch ligation in the prevention and control of postpartum hemorrhage after cesarean section.28 patients who underwent cesarean section and had a high risk of postpartum hemorrhage admitted to Liuzhi Special Zone Maternal and Child Health Hospital from July 2020 to September 2023 were selected as the study subjects,and they were randomly divided into a control group and a study group.The control group received preventive intervention with carboprost tromethamine injection solution in advance;The research group applied carboprost tromethamine injection and then performed uterine artery ascending branch ligation for treatment.The results showed that the incidence of postpartum hemorrhage in the study group was 4.76%,while the incidence of postpartum hemorrhage in the control group was 42.85%(P<0.05);The incidence of postoperative complications in the study group was 28.57%,while the incidence of postoperative complications in the control group was 35.71%(P>0.05);The study group had shorter surgical and hospitalization times,less intraoperative bleeding,and higher levels of Hb and RBC compared to the control group,with statistically significant differences(P<0.05);There was no statistically significant difference(P>0.05)in the recovery time of the first menstrual cycle and postpartum lochia between the two groups of patients after surgery.Research has found that the ligation of the ascending branch of the uterine artery is highly effective in the prevention and control of postpartum hemorrhage after cesarean section,and does not affect the patient’s ovarian,reproductive,or menstrual function.It has the value of promotion and application.
