纤溶酶联合硫辛酸治疗2型糖尿病周围神经病变的效果观察

摘　　要：目的 对2型糖尿病周围神经病变患者运用硫辛酸、纤溶酶联合治疗的效果进行探讨。方法 2型糖尿病周围神经病变患者65例,随机分为对照组(32例)和试验组(33例)。对照组采用二甲双胍片口服治疗,试验组采用硫辛酸、纤溶酶联合治疗。比较两组患者治疗效果、血糖(空腹血糖、餐后2 h血糖)水平、神经(正中神经、腓浅神经、腓总神经)传导速度、不良反应(胸闷、腹泻、恶心呕吐)发生情况。结果 试验组治疗总有效率93.94%高于对照组的68.75%(P<0.05)。治疗后,两组患者空腹血糖、餐后2 h血糖水平均降低,且试验组的空腹血糖(5.01±0.21)mmol/L、餐后2 h血糖(8.21±0.51)mmol/L较对照组的(6.12±0.33)、(10.63±0.55)mmol/L低(P<0.05)。治疗后,两组患者正中神经、腓浅神经、腓总神经传导速度较治疗前提升,且试验组正中神经传导速度(55.71±2.84)m/s、腓浅神经传导速度(45.87±4.11)m/s、腓总神经传导速度(42.84±3.21)m/s较对照组的(51.69±2.77)、(40.81±3.06)、(40.06±2.79)m/s高(P<0.05)。试验组不良反应发生率9.09%低于对照组的34.38%(P<0.05)。结论 对2型糖尿病周围神经病变患者采用硫辛酸联合纤溶酶治疗,可改善血糖水平,加快神经传导速度;同时也能减少不良反应,增强临床疗效,值得推广。Objective To explore the effect of fibrinolytic enzyme combined with lipoic acid on type 2 diabetes peripheral neuropathy.Methods 65 patients with type 2 diabetes peripheral neuropathy were randomly divided into a control group(32 cases)and an experimental group(33 cases).The control group was treated with metformin tablets orally,while the experimental group was treated with a combination of lipoic acid and fibrinolytic enzyme.Comparison was made on therapeutic effect,blood glucose(fasting blood glucose,2 h postprandial blood glucose)level,nerve(median nerve,superficial peroneal nerve,common peroneal nerve)conduction velocity,adverse reactions(chest tightness,diarrhea,nausea and vomiting)between the two groups.Results The total effective rate of the experimental group was 93.94%,which was higher than the control group's 68.75%(P<0.05).After treatment,the fasting blood glucose and 2 h postprandial blood glucose levels decreased in both groups;the fasting blood glucose and 2 h postprandial blood glucose in the experimental group were(5.01±0.21)and(8.21±0.51)mmol/L,which were lower than(6.12±0.33)and(10.63±0.55)mmol/L in the control group(P<0.05).After treatment,the nerve conduction velocity of median nerve,superficial peroneal nerve and common peroneal nerve in both groups increased compared with those before treatment;the nerve conduction velocity of median nerve in the experimental group was(55.71±2.84)m/s,the nerve conduction velocity of superficial peroneal nerve was(45.87±4.11)m/s,and the nerve conduction velocity of common peroneal nerve was(42.84±3.21)m/s,which were higher than(51.69±2.77),(40.81±3.06)and(40.06±2.79)m/s in the control group(P<0.05).The experimental group had lower incidence of adverse reactions of 9.09%than 34.38%in the control group(P<0.05).Conclusion Combined treatment of lipoic acid and fibrinolytic enzyme can improve blood glucose level and accelerate nerve conduction velocity in patients with type 2 diabetes peripheral neuropathy;At the same time,it can also reduce adverse re
