益胃汤加减治疗慢性萎缩性胃炎的系统评价与分析

摘　　要：目的系统评价益胃汤加减治疗慢性萎缩性胃炎(CAG)的临床疗效,并初步探索其潜在作用机制。方法计算机检索中国知网、万方数据库、中国生物医学文献数据库、维普数据库、PubMed、EMBase等数据库,时限为建库至2023年3月,运用RevMan 5.4软件进行Meta分析;利用TCMSP数据库、BATMAN-TCM数据库筛选益胃汤的化学成分及其作用靶点,利用GeneCards数据库、MalaCard数据库、DRUGBANK数据库以及pahrmgkb数据库检索与CAG相关的靶点,利用STRING数据库对交集靶点进行蛋白互作网络的构建并以degree值为标准筛选出核心靶点,利用David数据库对交集靶点进行GO生物分析和KEGG通路分析。结果入选文献13篇,患者1111例。Meta分析结果显示:与单用常规西药治疗相比,益胃汤联用常规西药临床疗效更好,不良反应更低。网络药理学分析得到有效成分25个,包括槲皮素、谷甾醇等,共同核心靶点90个,包括TP53、AKT、TNF、IL6等,相关核心通路15条,主要涉及癌症通路、AGE-RAGE通路、PI3K-Akt通路等。结论益胃汤加减联合常规西药可以提高CAG的临床疗效,其可能是通过保护胃黏膜、抑制炎症反应和防止胃癌的发生来达到治疗CAG的作用。Objective To systematically review the clinical efficacy of Yiwei Decoction in the treatment of Chronic Atrophic Gastritis,and to explore its potential mechanism.Methods The databases such as CNKI,Wanfang Database,China Biomedical Literature Database,VIP Database,PubMed,EMBase,etc were searched from the establishment of the database to March 2023.RevMan5.4 software was used for Meta-analysis.The TCMSP and BATMAN-TCM database were used to screen the chemical components and targets of Yiwei decoction.The database of GeneCards,MalaCard,DRUGBANK and pahrmgkb database were utilized to search for CAG-related targets.The STRING database was used to construct protein interactions networks for the intersecting targets and screen the core targets using the degree value as the criterion,and the David database was used to perform GO bioassays and KEGG pathway analyses for the intersecting targets.Results 13 studies involving 1111 patients were included.The results of Meta-analysis showed that compared with conventional Western medicine alone,Yiwei decoction combined with conventional Western medicine had better clinical efficacy and lower adverse reactions.Network pharmacological analysis revealed 25 active components,including quercetin and sitosterol,and 90 common core targets,including TP53,AKT,TNF,IL6,and 15 related core pathways,it mainly involves the AGE-RAGE,PI3K-Akt and MAPK pathways.Conclusion Modified Yiwei decoction combined with conventional Western medicine can improve the clinical efficacy of chronic Atrophic gastritis,it may protect the gastric mucosa,inhibit the inflammatory reaction and prevent the development of gastric cancer to treat chronic Atrophic gastritis.
