摘　　要：经典的颈椎后路手术包括半或全椎板切除术、椎板成形术、椎板扩大成形术等,但存在出血量大、并发症多、术后恢复慢等不足。近十年随着微创理念及内镜技术的普及,脊柱外科医生越来越关注微创术式治疗颈椎病。由最初的显微内窥镜发展到通道显微镜,再到近年来广泛应用的经皮内镜技术及双通道内镜技术。此外MRI神经根成像技术、超声引导脊神经阻滞术等诊断技术以及计算机导航辅助技术、全可视(镜外)环锯、镜下超声骨刀、镜下磨钻、镜下激光等技术的发展也为颈椎后路微创手术提供了便利。微创手术在有效减压的同时,也展现出创伤小、出血量低、术后恢复时间短等优势。本文对颈椎后路微创手术的治疗进展进行全面综述。Classic posterior cervical spine surgery includes hemilaminectomy or total laminectomy,laminoplasty,enlarged laminoplasty,etc.,but there are shortcomings such as large amount of blood loss,many complications,and slow postoperative recovery.In the past decade,with the popularization of minimally invasive concepts and endoscopic techniques,spine surgeons have paid more and more attention to minimally invasive surgery for the treatment of cervical spondylosis.The technologies have developed from the initial microendoscope to channel microscopy,and then to the widely used percutaneous endoscopy and dual-channel endoscopy technology in recent years.In addition,the development of diagnostic technologies such as MRI nerve root imaging technology,ultrasound-guided spinal nerve block,as well as computer-guided assisted technology,full-visualized trephine,microscopic ultrasonic bone knife,microscopic grinding and drilling,and microscopic laser also provides convenience for posterior minimally invasive surgery for cervical spondylosis.Minimally invasive surgery not only effectively decompresses,but also shows the advantages of less trauma,lower blood loss,and shorter postoperative recovery time.This article reviews the progress of posterior minimally invasive surgery for cervical spondylosis.
