右美托咪定对膝关节置换术患者麻醉疗效及短期睡眠的效果观察

摘　　要：目的:为提升膝关节置换术患者麻醉效果,降低麻醉影响,就右美托咪定麻醉药物的效果进行评价和分析。方法:选取2019年8月至2022年8月福建省三明市尤溪县中医医院收治的行膝关节置换术患者300例作为研究对象,按照随机数字表法分为对照组和观察组,每组150例。对照组采用利多卡因麻醉药物,观察组采用右美托咪定+利多卡因麻醉方案,比较2组的麻醉效果、睡眠质量、并发症等。结果:观察组麻醉后15 min、手术切皮后、术毕时镇静效果评分高于对照组,差异有统计学意义(P<0.05)。观察组患者匹兹堡睡眠质量指数低于对照组[(14.03±2.16)分],差异有统计学意义(P<0.05)。观察组麻醉后15 min、手术开始30 min、术毕时心率和平均动脉压低于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论:右美托咪定能够提升膝关节置换术麻醉效果,可以保证短期睡眠质量,对血流动力学影响小,有较好的推广性。Objective:To evaluate and analyze the efficacy of dexmedetomidine anesthesia in patients undergoing knee replacement surgery,to improve the anesthesia effect and reduce the impact of anesthesia.Methods:A total of 300 patients undergoing knee arthroplasty in our hospital from August 2019 to August 2022 were selected for this study,and were randomly divided into a control group(n=150,using lidocaine anesthesia)and an observation group(n=150,using dexmedetomidine+lidocaine anesthesia regimen)by simple randomization method.The anesthetic effect,sleep quality,and complications were compared between the two groups.Results:The sedative effect score of the observation group at 15 min after anesthesia and after the surgery was higher than that of the control group(P<0.05).The Pittsburgh Sleep Quality Index in the observation group was lower than the[(14.03±2.16)score]of the control group(P<0.05).Heart rate and mean arterial depression were observed at 15 min after anesthesia,30 min after surgery,and those in the control group(P<0.05).The overall incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:Dexmedetomidine can improve the anesthesia effect of knee replacement surgery,ensure short-term sleep quality,have little impact on hemodynamics,and have good generalizability.
