摘　　要：目的:分析小儿腹腔镜疝气手术麻醉诱导依从性的影响因素及与睡眠质量的关系。方法:选取2023年5月至2023年12月福建省漳州市医院收治的接受腹腔镜疝气手术的90例患儿作为研究对象,根据术前麻醉诱导期合作度量表(ICC)将患儿分为低依从组(ICC评分4~10分,n=32)和高依从组(ICC评分0~<4分,n=58)。采用多因素Logistic回归分析探究患儿麻醉诱导依从性的影响因素。结果:2组患儿的改良耶鲁术前焦虑量表(m-YPAS)评分、存在睡眠躁动、匹兹堡睡眠质量指数(PSQI)评分、睡眠时间、觉醒次数比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,m-YPAS评分(OR=1.728)、睡眠躁动(OR=1.642)、PSQI评分(OR=1.701)、睡眠时间(OR=1.664)、觉醒次数(OR=1.579)是腹腔镜疝气患儿术前麻醉依从性的影响因素(P<0.05)。结论:腹腔镜疝气手术麻醉诱导依从性受患儿术前的焦虑水平、术前睡眠躁动和睡眠质量的影响,睡眠质量越好,患儿对麻醉诱导的依从性越高。Objective:To analyze the influencing factors of anesthesia induction compliance in pediatric laparoscopic hernia surgery and its relationship with sleep quality.Methods:Ninety pediatric patients who underwent laparoscopic hernia surgery at Zhangzhou Hospital in Fujian Province from May 2023 to December 2023 were selected as the study subjects.According to the preoperative anesthesia induction cooperation scale(ICC),the patients were divided into a low compliance group(ICC score 4-10 points,n=32)and a high compliance group(ICC score 0-4 points,n=58).Using multiple logistic regression analysis to explore the influencing factors of anesthesia induction compliance in pediatric patients.Results:The modified Yale Preoperative Anxiety Scale(m-YPAS)score,presence of sleep agitation,Pittsburgh Sleep Quality Index(PSQI)score,sleep time,and number of awakenings were compared between the two groups of patients,and the differences were statistically significant(P<0.05).The results of multivariate logistic regression analysis showed that m-YPAS score(OR=1.728),sleep agitation(OR=1.642),PSQI score(OR=1.701),sleep time(OR=1.664),and number of awakenings(OR=1.579)were the influencing factors of preoperative anesthesia compliance in children with laparoscopic hernia(P<0.05).Conclusion:Compliance with anesthesia induction during laparoscopic hernia surgery is influenced by the preoperative anxiety level,preoperative sleep restlessness,and sleep quality of the patient.The better the sleep quality,the higher the patient's compliance with anesthesia induction.
