摘 要:目的 分析CXC类趋化因子配体5(CXCl-5)、可溶性白细胞分化抗原14亚型(sCD14-ST)、胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子-2(IGF-2)在急性细菌性脑膜炎患儿脑脊液中的表达及意义。方法 选取2020年7月-2021年7月湖州市中心医院收治的55例急性细菌性脑膜炎患儿为观察组,另选取同期在院进行健康体检的健康儿童45例为对照组。检测对照组血液及观察组血液、脑脊液中CXCl-5、sCD14-ST、IGF-1及IGF-2水平。分析CXCl-5、sCD14-ST、IGF-1及IGF-2在急性细菌性脑膜炎患儿脑脊液中表达及对急性细菌性脑膜炎的诊断价值。结果 与对照组相比,观察组血清、脑脊液中CXCl-5(36.88±11.23 vs. 50.27±5.38 vs. 67.55±20.37)ng/L、sCD14-ST(2.86±0.23 vs. 3.32±0.33 vs. 4.01±1.35)pg/ml、IGF-1(70.96±20.78 vs. 101.69±31.77 vs. 122.36±39.21)mg/L、IGF-2(65.37±18.65 vs. 102.33±27.88 vs. 121.58±37.69)mg/L水平较高(P<0.05);与观察组患儿血清相比,患儿脑脊液中CXCl-5(50.27±5.38 vs. 67.55±20.37)ng/L、sCD14-ST(3.32±0.33 vs. 4.01±1.35)pg/ml、IGF-1(101.69±31.77 vs. 122.36±39.21)mg/L、IGF-2(102.33±27.88 vs. 121.58±37.69)mg/L水平较高(P<0.05)。logistic回归分析显示CXCl-5、sCD14-ST、IGF-1及IGF-2水平升高是急性细菌性脑膜炎发生的影响因素。ROC曲线显示,4项联合对急性细菌性脑膜炎的诊断价值高于CXCl-5、sCD14-ST、IGF-1及IGF-2单项诊断(AUC=0.843、0.747、0.702、0.660、0.765),差异有统计学意义(P<0.05)。结论 急性细菌性脑膜炎患儿脑脊液中CXCl-5、sCD14-ST、IGF-1及IGF-2表达较高,CXCl-5、sCD14-ST、IGF-1及IGF-2联合对急性细菌性脑膜炎的诊断价值较高。Objective Analyze CXC class chemokine ligand 5(CXCl-5),soluble leukocyte differentiation antigen 14 subtype(sCD14-ST),and insulin-like growth factors The expression and significance of ICF-1 and IGF-2 in the cerebrospinal fluid of children with acute bacterial meningitis.Methods Select July 2020-In July 2021,55 children with acute bacterial meningitis admitted to Huzhou Central Hospital were selected as the observation group,and 45 healthy children who underwent health examinations in the hospital during the same period were selected as the control group Photo group.Detect the levels of CXCI-5,sCD14-ST,ICF-1,and ICF-2 in the blood of the control group and the blood and cerebrospinal fluid of the observation group.Analyze CXCI-5,sCD14-ST,IGF-1The expression of ICF-2 and its diagnostic value in cerebrospinal fluid of children with acute bacterial meningitis.Results Compared with the control group,the observation group had serum and cerebrospinal fluid CXCl-5 in liquid(36.88±11.23 vs.50.27±5.38 us.67.55±20.37)ng/L,sCD14-ST(2.86±0.23us.3.32±0.33 us.4.01±1.35)pg/ml ICF-1(70.96±20.78 vs.101.69±31.77 vs.122.36±39.21)mg/L,ICF-2(65.37±18.65 vs.102.33±27.88 us.121.58±37.69)mg/L The level of is relatively high(P<005);Compared with the serum of the observation group,CXCl-5(50.27±5.38 us.67.55±20.37)ng/L SCD14-ST(3.32±0.33 us.4.01±1.35)pg/ml,IGF-1(101.69±31.77 vs.122.36±39.21)mg/L,IGF-2(102.33±27.88 vs.121.58±37.69)mg/L.The level of is relatively high(P<0.05).Logistic regression analysis shows that elevated levels of CXCl-5,sCD14-ST,ICF-1 and ICF-2 are acute bacterial The influencing factors of men-ingitis occurrence.The ROC curve shows that the diagnostic value of four combinations for acute bacterial meningitis is higher than that of CXCl-5,sCD14-ST,ICF-1 and ICF-2 alone Diagnosis(AUC=0.843,0.747,0.702,0.660,0.765),the difference was statistically significant(P<0.05)..Conclusion In cerebrospinal fluid of children with acute bacterial meningitis CXCl-5,sCD14-ST,ICF-1 and IGF-2 are highly expressed,and the
