摘  要:目的探讨个案管理师结合数字化平台的延续护理模式对优化乳腺癌患者全程管理的临床应用价值。方法收集2022年1月13日至2023年1月13日期间于我院就诊的543例乳腺癌患者的相关资料。依据制订的患者管理计划、患者响应及完成情况来分析患者参与度;依据患者上传的问卷报告及复查指标来分析患者不良反应发生情况及临床管理效率。结果在为期1年的管理中,共制订患者管理计划9703次,累计随访105例患者并制订了297次随访任务,回访率为16.95%。227例患者累计填报问卷473份,31例患者上传复查指标共80份。患者脱落率为2.58%。29例患者发生问卷预警,共计33次;3例患者报告指标预警,共4次。个案管理师对问卷预警响应时间和指标预警响应时间分别为71.58 min和459.33 min。预警报告的3~4级不良反应主要为疼痛、食欲减退、便秘、中性粒细胞严重减低等。结论延续护理模式将有助于提高患者自我管理参与度,优化医务工作者临床管理效率,对未来乳腺癌患者临床管理新思路有一定启示。Objective To explore the clinical application value of continuous nursing mode combined with digital platform in optimizing the whole process management of patients with breast cancer.Methods Data of 543 patients with breast cancer admitted to our hospital from January 13,2022 to January 13,2023 were collected.The patient engagement,response and completion based on the developed patient management plan were analyzed;the occurrence of adverse reactions and clinical management efficiency were analyzed according to the questionnaire report uploaded by the patients and the review indicators.Results During the 1-year management,a total of 9703 patient management plans were formulated,105 patients were followed up and 297 follow-up tasks were formulated,with a return rate of 16.95%.227 patients filled in a total of 473 questionnaires,31 patients uploaded a total of 80 review indicators.Among them,the shedding rate was 2.58%.Questionnaire warning occurred in 29 patients,a total of 33 times.Three patients reported indicator warning,a total of 4 times.The response time of case managers for questionnaire warning and index warning was 71.58 minutes and 459.33 minutes,respectively.The main grade 3-4 adverse reactions reported were pain,loss of appetite,constipation,and severe neutrophil reduction.Conclusion The continuous nursing model will help to improve the participation of patients in self-management,optimize the efficiency of clinical management of medical workers,and have certain enlightenment for the future clinical management of breast cancer patients.
