老年肺炎患者血清CRP、IL-6、PCT水平变化及其临床意义

摘　　要：目的分析老年肺炎患者血清C反应蛋白(CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)水平变化及其临床意义。方法回顾性抽取2020年1月至2020年12月郑州大学第一附属医院收治的年龄>65岁的有呼吸道症状的患者255例,依据影像学表现分为肺炎组(152例)和非肺炎组(103例)。所有患者入院12 h内采用免疫比浊法检测血清CRP水平,采用电化学发光法检测IL-6、PCT水平。比较两组血清CRP、IL-6、PCT水平,采用受试者工作特征曲线下面积(AUC)分析三项指标对老年肺炎的诊断价值。结果肺炎组血清CRP、IL-6、PCT水平(32.40 mg/L、21.65 pg/mL、0.08 ng/mL)高于非肺炎组(2.56 mg/L、5.10 pg/mL、0.03 ng/mL),P<0.05。血清CRP和IL-6水平诊断肺炎的AUC(0.90、0.88)高于PCT(0.77),P<0.05。结论因呼吸道疾病入院的老年患者血清CRP和IL-6检测较PCT能更好地指导肺炎的诊断,尤其针对一些症状不明显的老年呼吸道疾病患者,应联合检测CRP、IL-6、PCT水平,以便及时准确判断病情。Objective To analyze the changes of levels of serum C-reactive protein(CRP),interleukin-6(IL-6)and procalcitonin(PCT)in elderly patients with pneumonia,and theirs clinical significance.Methods A total of 255 patients aged over 65 years with respiratory symptoms admitted to the First Affiliated Hospital of Zhengzhou University from January 2020 to December 2020 were retrospectively selected.And they were divided into the pneumonia group(152 cases)and the non-pneumonia group(103 cases)according to clinical imaging findings.The serum levels of CRP was detected by immunoturbidimetry and the levels of IL-6 and PCT were detected by electrochemiluminescence after admission.The serum levels of CRP,IL-6 and PCT were compared between the two groups.The area under the receiver operating characteristic curve(AUC)was used to analyze the diagnostic value of the three indicators in the diagnosis of pneumonia in the elderly.Results The levels of serum CRP,IL-6 and PCT in the pneumonia group(32.40 mg/L,21.65 pg/mL,0.08 ng/mL)were higher than those in the non-pneumonia group(2.56 mg/L,5.10 pg/mL,0.03 ng/mL),P<0.05.The AUC of CRP and IL-6(0.90,0.88)in the diagnosis of pneumonia was higher than that of PCT(0.77),P<0.05.Conclusions The detection of serum CRP and IL-6 in elderly patients admitted to the hospital for respiratory diseases can better guide the diagnosis of pneumonia than PCT.The combined detection of levels of CRP,IL-6 and PCT should be performed for elderly patients with respiratory diseases with obviousness symptoms,especially,to diagnose the state of illness timely and accurately.
