摘　　要：目的探讨小剂量艾司氯胺酮联合丙泊酚用于宫腔镜检查术的镇痛效果。方法观察性研究。抽取2020年8月至2022年12月于河南科技大学附属许昌市中心医院行宫腔镜检查术的患者98例,按照术中麻醉方案将其分为研究组和对照组。研究组40例,采用小剂量艾司氯胺酮复合丙泊酚麻醉;对照组58例,采用常规剂量艾司氯胺酮复合丙泊酚麻醉。比较两组麻醉和手术相关指标、不良反应发生情况及术后满意度评分。结果研究组苏醒时间短于对照组,术后改良Aldrete复苏评分高于对照组(P<0.05),但两组手术时间及丙泊酚用量比较差异未见统计学意义(P>0.05)。研究组术中不良反应总发生率(7.5%,3/40)低于对照组(24.1%,14/58),P<0.05。术前,两组炎性指标水平比较差异未见统计学意义(P>0.05);术后,两组炎性指标水平均高于术前,但研究组低于对照组(P<0.05)。研究组术后满意度评分高于对照组(P<0.05)。结论小剂量艾司氯胺酮联合丙泊酚用于宫腔镜检查术镇痛效果更佳,安全性更高,术后炎性反应较轻。Objective To investigate the analgesic effect of low-dose esketamine combined with propofol in hysteroscopy.Methods This study was an observational study.Ninety-eight patients who underwent hysteroscopy in Xuchang Central Hospital Affiliated to Henan University of Science and Technology from August 2020 to December 2022 were selected,and they were divided into study group and control group according to the intraoperative anaesthetic regimen.In the study group,40 patients were anesthetized with low-dose esketamine combined with propofol;in the control group,58 patients were anesthetized with conventional dose esketamine combined with propofol.The anesthesia and surgery-related indicators,adverse reactions,and postoperative satisfaction scores were compared between the two groups.Results The awakening time of the study group was shorter than that of the control group,while the postoperative modified Aldrete score was higher than that of the control group(P<0.05).There was no significant difference in the comparison of the operation time and propofol dosage between the two groups(P>0.05).The total incidence of intraoperative adverse reactions in the study group(7.5%,3/40)was lower than that in the control group(24.1%,14/58),P<0.05.There was no significant difference in levels of inflammatory indexes between the two groups before operation(P>0.05).After surgery,the levels of inflammatory indexes of the two groups were higher than those before operation,but the study group had lower levels of inflammatory indexes compared with the control group(P<0.05).The postoperative satisfaction score of the study group was higher than that of the control group(P<0.05).Conclusions Low-dose esketamine combined with propofol in hysteroscopy has better analgesic effect,higher safety and less postoperative inflammation.
