经尿道前列腺钬激光剜除术治疗小体积前列腺增生的效果和安全性

摘　　要：目的分析经尿道前列腺钬激光剜除术(HoLEP)治疗小体积前列腺增生(BPH)的效果及安全性。方法回顾性抽取2020年1月至2023年1月郑州大学第一附属医院收治的BPH患者102例,依据术式分为对照组(51例)和研究组(51例)。对照组行经尿道前列腺电切术治疗,研究组行HoLEP治疗。比较两组围术期指标、尿流动力学指标、前列腺症状、性功能、生活质量及并发症发生情况。结果研究组手术时间、膀胱冲洗时间及导尿管留置时间短于对照组,研究组术中出血量少于对照组,差异有统计学意义(t=9.52、14.99、7.19、16.15,P均<0.05)。术后,研究组最大尿流率、膀胱顺应性、国际勃起功能指数问卷表-5评分和BPH患者生活质量测定量表评分高于对照组,排尿后残余尿量、国际前列腺症状评分低于对照组,差异有统计学意义(t=14.78、9.76、12.80、5.39、19.74、6.16,P均<0.05)。研究组并发症发生率(7.84%,4/51)低于对照组(23.53%,12/51),P<0.05。结论HoLEP治疗小体积BPH可促进患者尿流动力学改善,缩短术后恢复时间,且并发症少。Objective To analyze the efficacy and safety of transurethral holmium laser enucleation of the prostate(HoLEP)in the treatment of small-volume benign prostatic hyperplasia(BPH).Methods A total of 102 patients with small-volume BPH admitted to the First Affiliated Hospital of Zhengzhou University from January 2020 to January 2023 were retrospectively selected,and the patients were divided into the control group(51 cases)and the study group(51 cases)according to surgical procedures.The control group was treated by transurethral resection of the prostate,while the study group was treated by HoLEP.The perioperative indexes,urodynamics indexes,prostate symptoms,sexual function,quality of life and complications were compared between the two groups.Results The surgical duration,length of bladder irrigation,and length of catheter retention in the study group were shorter than those in the control group,and the intraoperative bleeding in the study group was less than that in the control group(t=9.52,14.99,7.19,16.15;all P<0.05).After surgery,the maximum flow rate,bladder compliance,international index of erectile function-5 scores,and quality of life scale score in the study group were significantly higher than those in the control group,moreover,the post-void residual and international prostate symptom scores in the study group were significantly lower than those in the control group(t=14.78,9.76,12.80,5.39,19.74,6.16;all P<0.05).The incidence of complications in the study group(7.84%,4/51)was significantly lower than that in the control group(23.53%,12/51),P<0.05.Conclusions The application of HoLEP in the treatment of small-volume BPH can improve urodynamics in patients,and shorten the postoperative recovery time,with fewer complications.
