摘　　要：目的比较经导管主动脉瓣置入术(TAVI)与主动脉瓣置换术(SAVR)治疗重度主动脉瓣狭窄(SAS)的临床效果。方法回顾性抽取2018年1月至2022年6月平顶山市第一人民医院收治的SAS患者102例,依据治疗方法分为对照组(51例,采用SAVR治疗)和观察组(51例,采用TAVI治疗)。比较两组手术情况、术后6个月心功能及并发症发生情况。结果对照组术中出血量大于观察组,手术时间、主动脉阻断时间短于观察组,住院时间长于观察组(P<0.05)。术后6个月,两组心功能分级情况比较差异未见统计学意义(P>0.05)。对照组再入院发生率高于观察组(P<0.05);两组新发心房颤动、脑卒中、房室传导阻滞发生率比较,差异未见统计学意义(P>0.05)。结论TAVI、SAVR治疗均可改善SAS患者心功能,但TAVI可减少术中出血量,缩短康复时间,降低再入院率。Objective To compare the therapeutic effects of transcatheter aortic valve implantation(TAVI)versus surgical aortic valve replacement(SAVR)on severe aortic valve stenosis(SAS).Methods A total of 102 SAS patients admitted to the Pingdingshan First People’s Hospital from January 2018 to June 2022 were retrospectively selected,and they were divided into control group(51 cases,treated by SAVR)and observation group(51 cases,treated by TAVI)according to the treatment methods.The operation,cardiac function 6 months after surgery,and incidence of complications were compared between the two groups.Results The control group had more intraoperative bleeding,shorter operation time,shorter aortic occlusion time,and longer hospital stay compared with the observation group(P<0.05).There was no significant difference in classification of cardiac function between the two groups 6 months after surgery(P>0.05).The incidence of readmission in the control group was higher than that in the observation group(P<0.05).There was no significant difference in the incidence of new-onset atrial fibrillation,stroke,and atrioventricular block between the two groups(P>0.05).Conclusions Both TAVI and SAVR can improve the cardiac function of SAS patients.However,TAVI treatment can reduce intraoperative bleeding,shorten recovery time,and lower readmission rate.
