多次输血患者检测Rh分型的临床意义

摘　　要：目的探讨多次输血患者检测Rh分型的临床意义。方法前瞻性研究。抽取2020年3月至2022年3月平煤神马医疗集团总医院输血科检测出阳性抗体的110例患者作为研究对象。使用微柱凝胶法检测110例患者的特异抗体占比情况;使用微柱凝胶法检测110例阳性患者的抗体类型,使用盐水试管法与抗球蛋白法检测阳性患者的交叉配血符合率。结果110例阳性患者抗体检测中,共有71例(64.55%)Rh系统患者,其中有抗E型65例(59.09%),抗c型6例(8.45%);MNSs血型系统共12例(10.91%),其中抗M 9例(8.18%),抗Mur 3例(2.73%);Lewis系统共13例(11.82%);P系统3例(2.73%);其他非特异性抗体1例(0.91%)。Rh分型结果按占比从高到低排列为:DCCee 67例(60.91%),DCcEE 28例(25.45%),DCeee 8例(7.27%),DccEE 4例(3.64%),DccEe 1例(0.91%),dCcee 1例(0.91%),dccee 1例(0.91%);交叉配血结果显示,110例阳性患者配血率为93.64%(103/110),输血有效率为100%(110/110),配血时间为(31.24±2.31)min。结论对多次输血患者进行Rh分型检测,可有效检测出患者体内存在的多种抗体,避免患者产生不规则抗体,增加配血与输血的有效率。Objective To investigate the clinical significance of Rh antigen typing in patients with multiple blood transfusion.Methods This study was a prospective study.A total of 110 patients with positive antibodies detected in the Blood Transfusion Department of General Hospital of Pingmei Shenma Medical Group from March 2020 to March 2022 were selected as the research objects.The proportion of specific antibodies in 110 patients were detected by microcolumn gel method.The types of antibody in the 110 positive patients were detected by microcolumn gel method.The cross matching coincidence rate of positive patients was detected by saline test tube method and antiglobulin method.Results A total of 71 patients(64.55%)with Rh blood system were detected out among the 110 positive patients,including 65 cases(59.09%)anti-E type and 6 cases(8.45%)anti-c type;there were 12 cases(10.91%)with MNSs blood group system,including 9 cases(8.18%)with anti-M and 3 cases(2.73%)with anti-Mur.There were 13 cases(11.82%)with Lewis system,3 cases(2.73%)with P system and 1 case(0.91%)with other non-specific antibodies.The results of proportion of Rh antigen typing arranged from high to low as the following order:67 cases(60.91%)of DCCee,28 cases(25.45%)of DCcEE,8 cases(7.27%)of DCeee,4 cases(3.64%)of DccEE,1 case(0.91%)of DccEE,1 case(0.91%)of dCcee,and 1 case(0.91%)of dccee.The results of cross matching showed that the matching rate of 110 positive patients was 93.64%(103/110),the effective rate of blood transfusion was 100%(110/110),and the matching time was(31.24±2.31)min.Conclusions Rh antigen typing detection for patients with multiple blood transfusions can effectively detect a variety of antibodies in patients,avoid irregular antibodies in patients,and increase the efficiency of blood matching and blood transfusion.
