某三甲医院手术并发症与非计划再次手术关联性的探讨

摘　　要：目的分析某三甲医院手术并发症及非计划再次手术分布特征,为持续提升围手术期质量、保证医疗安全提供参考。方法对某三甲医院2022年1月1日-2022年12月31日发生的手术并发症情况进行回顾性分析,包含发生手术并发症的科室、手术并发症、非计划再次手术等内容。结果共开展手术12651例,发生手术并发症203例,总发生率为1.60%;非计划再次手术60例,总发生率为4.74‰。手术并发症发生率按发生科室排名前5位分别为是胸外科、神经外科、普外科、骨科和肝胆外科。手术并发症种类按构成比排名前5的依次为深静脉血栓、切口感染、败血症、肺部感染、植入物并发症及术后出血。通过斯皮尔曼统计分析,手术并发症发生率与非计划再次手术率呈线性正相关(r s=0.73),差异有统计学意义(P<0.05)。结论胸外科、神经外科、普外科、骨科等手术并发症较多,发生的并发症以深静脉血栓、切口感染、败血症、肺部感染、植入物并发症及术后出血等多见。手术并发症一定程度上影响非计划再次手术的发生。Objectives This study aims to analyze the distribution characteristics of surgical complications and unplanned reoperations in a tertiary hospital to provide a reference for continuously improving perioperative quality and ensuring medical safety.Methods Surgical complications that occurred in a tertiary hospital from January 1,2022 to December 31,2022 were retrospectively analyzed,including the departments where surgical complications occurred,surgical complications,unplanned re-operations,etc.Results A total of 12,651 surgeries were performed,and 203 surgical complications occurred,with an overall incidence rate of 1.60%.There were 60 cases of unplanned reoperation,and the overall incidence rate was 4.74‰.The top five departments in terms of incidence of surgical complications were thoracic surgery,neurosurgery,general surgery,orthopedics,and hepatobiliary surgery departments.The top five types of surgical complications in terms of composition ratio were deep vein thrombosis,incision infection,sepsis,pulmonary infection,implant complications,and postoperative bleeding.Through Spearman statistical analysis,there was a positive linear correlation between the incidence of surgical complications and the rate of unplanned reoperation(rs=0.73),and the difference was statistically significant(P<0.05).Conclusions There are many surgical complications in thoracic surgery,neurosurgery,general surgery,orthopedics,etc.The most common types include deep vein thrombosis,incision infection,sepsis,pulmonary infection,implant complications,and postoperative bleeding.Surgical complications affect the occurrence of unplanned reoperation to a certain extent.
