北京市某三级医院恶性肿瘤住院患者费用及其影响因素分析

摘　　要：目的探讨某三级医院恶性肿瘤住院患者费用构成及其影响因素,为恶性肿瘤患者出院费用能有效的控制提供参考。方法选取北京市某医院病案系统2022年1月1日-2022年12月31日出院3166例住院恶性肿瘤患者的总费用及构成情况,采用单因素方差分析和多元逐步回归分析法对住院费用的影响因素进行统计分析。结果3166例恶性肿瘤出院患者人均住院费用为32097.45元,西药费占比居于首位,达到38.25%;手术费和治疗费次之,分别占比36.41%和25.34%。前8名的恶性肿瘤住院费用占比达到47.15%。多因素分析结果显示,是否手术、疾病转归、性别、住院天数是影响恶性肿瘤住院患者住院费用的主要影响因素,其中,手术对出院费用影响最大,比较手术和非手术治疗的出院患者,手术患者平均住院费用增加8322.23元,均P<0.05。结论恶性肿瘤出院患者住院费用较高,其产生的经济负担较重,医院要积极制定政策,合理科学地控制医疗费用的支出,提高医院的管理水平和工作效率。Objectives This study aims to explore the composition and influencing factors of the cost of malignant tumor inpatients in a tertiary hospital and to provide a reference for effective control of the discharge cost of malignant tumor patients.Methods The total cost and composition of 3166 inpatients with malignant tumors discharged from the medical record system of a hospital in Beijing from January 1 to December 31,2022 were selected.The influencing factors of hospitalization cost were statistically analyzed by one-way ANOVA and multiple stepwise regression analysis.Results The per capita hospitalization expenses for 3,166 discharged patients with malignant tumors were 32,097.45 yuan,with Western medicine expenses accounting for the highest proportion,reaching 38.25%.Surgery fees and treatment fees followed,accounting for 36.41%and 25.34%respectively.The hospitalization expenses of the top 8 malignant tumors accounted for 47.15%.The results of the multifactor analysis showed that surgery,disease outcome,gender,and length of stay were the main factors affecting the hospitalization costs of inpatients with malignant tumors.Among them,surgery had the greatest impact on discharge costs.Comparing the discharged patients with surgical and non-surgical treatment,the average hospitalization cost of surgical patients increased by 8322.23 yuan,with both P<0.05.Conclusions The hospitalization cost of discharged patients with malignant tumors is high,and the economic burden is heavy.Hospitals should actively formulate policies,reasonably and scientifically control medical expenditure,and improve the management level and work efficiency of the hospital.
