摘 要:目的:探讨多糖铁复合物联合维生素D治疗妊娠期缺铁性贫血的效果。方法:回顾性选取2021年1月—2023年1月东莞市莞城医院收治的223例妊娠期缺铁性贫血患者。根据治疗方法的不同将其分为对照组(112例)和研究组(111例)。对照组给予多糖铁复合物,研究组在对照组基础上给予维生素D。比较两组临床疗效,治疗前、治疗3个月后血常规指标、铁代谢指标及不良反应。结果:研究组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗3个月后,研究组血红蛋白(Hb)、平均红细胞体积(MCV)及红细胞计数(RBC)均高于对照组,促红细胞生成素(EPO)低于对照组,差异有统计学意义(P<0.05)。治疗3个月后,研究组血清铁(SI)和转铁蛋白饱和度(TAST)均高于对照组,转铁蛋白(TRF)低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:在妊娠期缺铁性贫血中,补充维生素D可改善患者贫血情况、血常规及铁代谢指标,利于降低EPO水平,且应用的安全性较好。Objective:To explore the effect of polysaccharide iron complex combined with vitamin D in the treatment of iron deficiency anemia during pregnancy.Method:A total of 223 cases of iron deficiency anemia during pregnancy treated in Dongguan City Guancheng Hospital from January 2021 to January 2023 were retrospectively selected.They were divided into control group(112 cases)and study group(111 cases)according to different treatment method.The control group was given polysaccharide iron complex,and the study group was given vitamin D on the basis of the control group method.The clinical efficacy,blood routine indexes,iron metabolism indexes before treatment and after 3 months of treatment and adverse reactions of the two groups were compared.Result:The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After 3 months of treatment,hemoglobin(Hb),mean red blood cell volume(MCV)and red blood cell count(RBC)in the study group were higher than those in the control group,and erythropoietin(EPO)was lower than that in the control group,the differences were statistically significant(P<0.05).After 3 months of treatment,the serum iron(SI)and transferrin saturation(TAST)of the study group were higher than those of the control group,and the transferrin(TRF)of the study group was lower than that of the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:In iron deficiency anemia during pregnancy,supplementing vitamin D can improve the anemia situation of patients,blood routine and iron metabolism indicators,which is beneficial for reducing EPO levels and has good safety in application.
