摘 要:目的探讨妊娠期糖尿病(gestational diabetes mellitus,GDM)胎儿体重与母血、新生儿脐血微量元素含量的关系。方法构建GDM胎儿体重队列,根据入组和排除标准选择2022年3~8月南京市妇幼保健院因非病理性手术指征剖宫产分娩的产妇,分为正常对照组27例(C组)、GDM组38例(G组),包括GDM适龄儿组19例(GA组)和GDM巨大儿组19例(GM组),比较队列前期检测的所有母体外周血及新生儿脐静脉血9种微量元素的含量,探讨GDM胎儿体重与微量元素的相关性。结果与C组相比,G组产妇血清砷含量减少;脐血中锌、砷元素减少。与GA组相比,GM组产妇血清中硒元素含量减少,铅元素含量增加;脐血中砷元素增加。结论母体血清中砷、硒元素含量的减少与GDM和巨大儿的发生发展可能有关。孕期适当补充部分微量元素有望降低GDM发病率,减少分娩GDM巨大儿的风险。Objective To explore the relationship between fetal growth weights and the content of trace elements in maternal blood and neonatal cord blood in pregnant women with gestational diabetes mellitus(GDM).Methods The GDM fetal weight cohort was constructed.According to the inclusion and exclusion criteria,selected postpartum women who underwent cesarean section due to non pathological surgical indications at Nanjing Maternal and Child Health Hospital from March to August 2022,27 cases in the normal control group(group C)and 38 cases in the GDM group(group G),including 19 cases in the GDM age-appropriate infants group(group GA)and 19 cases in the GDM macrosomia group(group GM).The contents of 9 trace elements in all maternal peripheral blood and newborn umbilical venous blood detected at the earlier stage of the cohort were compared to explore the correlation between GDM fetal weight and trace elements.Results Compared with group C,the serum arsenic content in group G postpartum women decreased;the zinc and arsenic elements in cord blood decreased.Compared with the group GA,the selenium content in the serum of mothers in the group GM decreased while the lead content increased;and arsenic increased in cord blood.Conclusion The decrease in arsenic and selenium content in maternal serum may be related to the occurrence and development of GDM and macrosomia.Appropriate supplementation of some trace elements during pregnancy is expected to reduce the incidence rate of GDM and reduce the risk of delivery of GDM macrosomia.
