玉女煎加减治疗胃热炽盛证2型糖尿病临床研究

摘　　要：目的:观察玉女煎加减对胃热炽盛证2型糖尿病(T2DM)患者血糖、胰岛素β细胞功能的影响。方法:选取106例胃热炽盛型T2DM患者,按随机数字表法分为试验组及对照组各53例。对照组采用常规治疗,试验组在此基础上采用玉女煎加减治疗。比较2组临床疗效,比较2组治疗前后胃热炽盛型证候评分、血糖指标值[空腹血糖(FBG)、餐后2 h血糖(P2hBG)、糖化血红蛋白(HbA1c)]、胰岛β细胞功能指标值[空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)]的变化。结果:试验组临床疗效总有效率为94.34%,对照组为77.36%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组胃脘灼热、口臭、口渴嗜冷、次症、总分均较治疗前下降(P<0.05),试验组上述4项评分及总分均低于对照组(P<0.05)。治疗后,2组FBG、P2hBG、HbA1c指标值均较治疗前下降(P<0.05),试验组FBG、P2hBG、HbA1c指标值均低于对照组(P<0.05)。治疗后,2组FINS、HOMA-IR指标值均较治疗前下降(P<0.05),HOMA-β指标值均较治疗前上升(P<0.05);试验组FINS、HOMA-IR指标值均低于对照组(P<0.05),HOMA-β指标值高于对照组(P<0.05)。结论:玉女煎加减治疗胃热炽盛型T2DM疗效显著,能有效缓解临床症状,降低血糖水平,改善胰岛β细胞功能。Objective:To observe the effect of modified Yunv Jian on blood glucose and pancreaticβcell function in patients with type 2 diabetes mellitus(T2DM)with intense stomach heat syndrome.Methods:A total of 106 T2DM patients with intense stomach heat syndrome were selected and divided into the trial group and the control group according to the random number table method,with 53 cases in each group.The control group received routine treatment,and the trial group received modified Yunv Jian based on the treatment of the control group.The clinical effects were compared between the two groups.Before and after treatment,the changes in scores of intense stomach heat syndrome,blood glucose indicators[fasting blood glucose(FBG),postprandial 2-hour blood glucose(P2hBG),and hemoglobin A1c(HbA1c)],and pancreaticβcell function indexes[fasting serum insulin(FINS),Homeostatic Model Assessment of Insulin Resistance(HOMA-IR),and Homeostasis Model Assessment ofΒ-Cell Function(HOMA)-β)]were compared between the two groups.Results:The total clinical effective rate was 94.34%in the trial group,and 77.36%in the control group,the difference being significant(P<0.05).After treatment,the scores of epigastric burning,bad breath,thirst and coldness,secondary symptoms,and total scores in the two groups were decreased when compared with those before treatment(P<0.05).The scores of the above four items and total score in the trial group were lower than those in the control group(P<0.05).After treatment,the values of FBG,P2hBG,and HbA1c in the two groups were decreased when compared with those before treatment(P<0.05),and the values of FBG,P2hBG,and HbA1c in the trial group were lower than those in the control group(P<0.05).After treatment,the values of FINS and HOMA-IR in the two groups were decreased when compared with those before treatment(P<0.05),and HOMA-βvalues were increased when compared with those before treatment(P<0.05);the values of FINS and HOMA-IR in the trial group were lower than those in the control group(P<0.05),and HOMA-βv
