三子清肺汤治疗痰热壅肺型慢性阻塞性肺病急性加重期临床研究

摘　　要：目的:观察三子清肺汤治疗慢性阻塞性肺病急性加重期(AECOPD)痰热壅肺型的临床疗效。方法:选取120例AECOPD患者为研究对象,按随机数字表法分为研究组及对照组各60例。对照组进行常规现代医学治疗,研究组在对照组基础上给予三子清肺汤治疗,2组均连续治疗2周。比较2组临床疗效及不良反应发生情况,比较2组治疗前后中医证候积分、炎症反应[C-反应蛋白(CRP)、白细胞介素-6 (IL-6)、肿瘤坏死因子-α (TNF-α)]指标值的变化。结果:治疗后,2组咳嗽气喘、痰多稠黄、胸痛胸闷、烦热口干、大便干结中医证候积分及总分均较治疗前下降(P<0.05),研究组上述5项积分及总分均低于对照组(P<0.05)。对照组临床疗效总有效率为83.33%,研究组为95.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组CRP、IL-6、TNF-α水平均较治疗前下降(P<0.05),研究组CRP、IL-6、TNF-α水平均低于对照组(P<0.05)。2组治疗过程中均未发生明显药物不良反应。结论:三子清肺汤治疗痰热壅肺型AECOPD疗效较好,可改善肺通气,抑制炎症反应。Objective:To observe the clinical effect of Sanzi Qingfei Decoction on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)of phlegm-heat obstructing the lung type.Methods:A total of 120 AECOPD patients were selected as the study objects and were divided into the study group and the control group according to the random number table method,with 60 cases in each group.The control group was given conventional modern medical treatment,and the study group was additionally given Sanzi Qingfei Decoction based on the treatment of the control group.Both groups were treated for 2 weeks.The clinical effects and the occurrence of adverse reactions were compared between the two groups,and the changes of traditional Chinese medicine(TCM)syndrome scores,inflammatory response[C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]were also compared before and after treatment.Results:After treatment,the TCM syndrome scores of cough and asthma,excessive,thick and yellow phlegm,chest pain and oppression,vexing heat and dry mouth,stool dryness and accumulation,and the total scores in the two groups were decreased when compared with those before treatment(P<0.05),and the above five scores and the total score in the study group were lower than those in the control group(P<0.05).The total effective rate was 83.33%in the control group and 95.00%in the study group, the difference being significant (P<0.05). After treatment, the levels of CRP, IL-6, andTNF-α in the two groups were decreased when compared with those before treatment (P<0.05),and thelevels of CRP,IL-6,and TNF-α in the study group were lower than those in the control group (P<0.05).No obvious adverse drug reactions occurred in the two groups during the treatment. Conclusion: SanziQingfei Decoction has a good curative effect on AECOPD of phlegm-heat obstructing the lung type,whichcan improve pulmonary ventilation and inhibit inflammatory reaction.
