摘　　要：目的:为探讨超声引导下针刀疗法治疗膝骨关节炎的临床疗效。方法:将50例膝骨关节炎的患者随机分为观察组与对照组,各25例。观察组接受超声引导下针刀疗法,每周1次共5周;对照组接受传统针刀治疗,每周1次共5周。在治疗第2周和治疗结束时以及结束后间隔7周,分别比较两组治疗前后视觉模拟评分法(VAS)评分、西安大略和麦克马斯特大学骨关节炎指数评分(WOMAC)、膝关节Lysholm评分系统评分及焦虑自评量表(SAS)得分。结果:干预第2周观察组各项评分明显优于对照组,差异具有统计学意义(P<0.01);干预结束时观察组关节功能评分、SAS评分低于对照组,差异具有统计学意义(P<0.05);而其他评分比较差异无统计学意义(P>0.05);7周后随访时观察组各项评分优于对照组,差异具有统计学意义(P<0.01)。结论:与传统针刀疗法比较,超声引导下针刀疗法可以快速有效地缓解患者的疼痛焦虑症状、改善关节功能且远期疗效显著。Objective:To explore the clinical efficacy of ultrasound-guided acupotomy therapy in treatment of knee osteoarthritis(KOA).Methods:50 patients with KOA were randomly assigned into the observation group and the control group,with 25 patients in each group.The observation group received ultrasound-guided acupotomy therapy,once a week for 5 weeks;whereas the control group received traditional acupotomy therapy,once a week for 5 weeks.Scores of Visual Analog Scale(VAS),Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC),Lysholm knee assessment and Self-Rating Anxiety Scale(SAS)were compared between the two groups at the second week of treatment,at the end of treatment,and at 7-week follow-up after the treatment.Results:At the second week of treatment,the observation group showed significantly better scores compared to the control group(P<0.01).There were no significant differences in scores between the observation group and the control group at the end of treatment(P>0.05).However,at the 7-week follow-up visit,the observation group demonstrated superior scores compared to the control group(P<0.01).Conclusion:Ultrasound-guided acupotomy therapy is more effective than traditional acupotomy therapy in rapidly relieving pain and anxiety symptoms,improving joint function,and achieving significant long-term efficacy for patients with KOA.
