中成药对原发性肝癌的治疗研究进展

摘　　要：原发性肝癌是常见的难治性肿瘤,且中晚期的肝癌患者预后差,生存质量低下且复发率高,病死率居于中国恶性肿瘤第二位,严重威胁着慢性肝病患者的生命安全。中医药治疗原发性肝癌具有一定优势,其中,中成药具有方便服用、依从性好等优点,拥有广阔的临床应用前景。中成药在改善原发性肝癌患者症状,提高患者生存质量,降低不良反应,提高联合治疗效率,延长患者生存时间,以及降低原发性肝癌复发率等方面具有积极意义。机制上,中成药可通过调节肿瘤细胞的内外环境从而治疗原发性肝癌,即调控肿瘤免疫微环境及肠道菌群,调控肿瘤代谢模式并抑制肿瘤细胞的生长和增殖,调控肝癌细胞分子修饰并抑制肿瘤细胞的侵袭和转移,以及诱导肿瘤细胞死亡。该文整理了对原发性肝癌具有潜在治疗作用的中成药,综合阐述并概括了相关临床研究和实验研究结果,归纳出中成药治疗原发性肝癌的机制特点,以期为今后中药治疗肝癌策略探索和新药开发提供思路。Primary liver cancer,a prevalent refractory tumor,has poor prognosis,low quality of life and high recurrence rate in the patients in advanced stage.With the second mortality rate in China,primary liver cancer seriously threatens the life safety of patients with chronic liver disease.Traditional Chinese medicine has demonstrated advantages in the treatment of primary liver cancer.Chinese patent medicine boasts the advantages of convenient administration and high compliance,rendering it a promising candidate for extensive clinical applications.It possesses positive advantages in the management of primary liver cancer,particu-larly in ameliorating symptoms and enhancing quality of life,mitigating toxicities and side effects,augmenting the efficacy of com-bined therapy,prolonging patient survival time and reducing recurrence rates.Mechanistically,Chinese patent medicine can regu-late the internal and external environment of tumor cells,including the immune microenvironment and intestinal flora,metabolic patterns,growth and proliferation of tumor cells,molecular modification of liver cancer cells,invasion and metastasis inhibition as well as induction of tumor cell death.This article summarized the potential therapeutic effects of Chinese patent medicines on pri-mary liver cancer.Moreover,it summarized the mechanism and characteristics of Chinese patent medicines in primary liver canc-er treatment from relevant clinical and experimental research reports.It is expected to provide an idea for the exploration of tradi-tional Chinese medicine treatment strategies and new drug development for primary liver cancer therapy.
