摘 要:目的探讨血清β-连环蛋白(β-catenin)、E-钙黏蛋白(E-cadherin)水平与老年冠心病患者冠状动脉狭窄的关系。方法将2021年7月至2023年7月收治的206例老年冠心病患者作为研究对象(观察组),根据冠状动脉狭窄程度分为轻度狭窄组(Gensini评分≤24分,n=122)、中重度狭窄组(Gensini评分>24分,n=84)2个亚组。另外选取同期进行体检的250例健康人作为对照组。采用酶联免疫吸附法检测血清β-catenin、E-cadherin水平;采用Pearson相关分析探讨老年冠心病患者血清β-catenin、E-cadherin水平与Gensini评分的相关性;采用多因素logistic逐步回归分析探讨老年冠心病患者冠状动脉狭窄程度的影响因素。结果观察组血清β-catenin水平高于对照组,E-cadherin水平低于对照组(P均<0.05)。中重度狭窄组血清β-catenin水平高于轻度狭窄组,E-cadherin水平低于轻度狭窄组(P均<0.05)。Pearson相关分析结果显示,血清β-catenin与Gensini评分呈正相关,血清E-cadherin与Gensini评分呈负相关(P均<0.05)。中重度狭窄组患者年龄、吸烟占比、糖尿病史占比、白细胞计数、LDL-C大于轻度狭窄组,HDL-C小于轻度狭窄组(P均<0.05)。多因素logistic逐步回归分析显示,有糖尿病史(OR=2.333,95%CI:1.558~3.493)、β-catenin≥2.51 pg/mL(OR=3.133,95%CI:2.012~4.879)、E-cadherin≤3.61 ng/mL(OR=2.625,95%CI:1.702~4.048)是老年冠心病患者冠状动脉狭窄程度的危险因素,HDL-C(OR=0.467,95%CI:0.320~0.680)是老年冠心病患者冠状动脉狭窄程度的保护因素(P均<0.05)。结论老年冠心病患者血清β-catenin水平升高、E-cadherin水平降低,两者与冠状动脉狭窄程度密切相关,且血清β-catenin水平升高、E-cadherin水平降低是老年冠心病患者冠状动脉狭窄程度的危险因素。Objective To explore the relationship between serum levels ofβ⁃catenin and E⁃cadherin and coronary artery stenosis in elderly patients with coronary heart disease.Methods A total of 206 elderly patients with coronary heart disease admitted to our hospital from July 2021 to July 2023 were selected as the research objects(observation group).According to the degree of coronary artery stenosis,they were divided into mild stenosis group(Gensini score≤24,n=122)and moderate to severe stenosis group(Gensini score>24,n=84).In addition,250 healthy people who underwent physical examination during the same period were selected as the control group.The levels of serumβ⁃catenin and E⁃cadherin were detected by enzyme⁃linked immunosorbent assay.Multivariate Logistic stepwise regression analysis was used to explore the influencing factors of the degree of coronary artery stenosis in elderly patients with coronary heart disease.Results The serumβ⁃catenin level in observation group was higher than that in control group,and the serum E⁃cadherin level was lower than that in control group(all P<0.05).The serumβ⁃catenin level in moderate and severe stenosis group was higher than that in mild stenosis group,and the serum E⁃cadherin level was lower than that in mild stenosis group(all P<0.05).Age,smoking rate,diabetes history rate,white blood cell count and LDL⁃C in moderate to severe stenosis group were higher than those in mild stenosis group,and HDL⁃C was lower than those in mild stenosis group(all P<0.05).History of diabetes(OR=2.333,95%CI:1.558⁃3.493),β⁃catenin≥1.22 pg/mL(OR=3.133,95%CI:2.012⁃4.879),E⁃cadherin≤3.61 ng/mL(OR=2.625,95%CI:1.702~4.048)were risk factors for coronary artery stenosis degree in elderly patients with coronary heart disease,and HDL⁃C(OR=0.467,95%CI:0.320⁃0.680)was a protective factor for the degree of coronary artery stenosis in elderly patients with coronary heart disease(all P<0.05).Conclusion The increase of serumβ⁃catenin level and the decrease of E⁃cadher
