依托咪酯复合盐酸氢吗啡酮超前镇痛用于无痛人工流产手术的效果分析

摘　　要：目的探讨盐酸氢吗啡酮超前镇痛复合依托咪酯麻醉方案用于无痛人工流产手术的临床价值。方法前瞻性随机选取2022年5月—2023年5月福建中医药大学附属第三人民医院妇科收治的180例择期行无痛人工流产的早期妊娠妇女为研究对象,经随机数表法将其分为对照组和观察组,各90例。对照组采用依托咪酯进行麻醉,观察组在对照组的基础上增用盐酸氢吗啡酮进行麻醉。比较两组心率与平均动脉压、术后宫缩痛评分、苏醒时间、不良反应发生情况。结果观察组术后宫缩痛评分为(0.80±0.31)分,低于对照组的(1.43±0.57)分,差异有统计学意义(t=9.211,P<0.001)。观察组不良反应的总发生率为3.33%,低于对照组的11.11%,差异有统计学意义(χ^(2)=4.063,P=0.044)。两组不同时间节点的平均动脉压、心率以及苏醒时间对比,差异无统计学意义(P均>0.05)。结论在无痛人工流产手术中,盐酸氢吗啡酮复合依托咪酯的麻醉效果较好,可改善术后宫缩痛,降低麻醉相关不良反应风险。Objective To investigate the clinical value of preanalgesia with hydromorphone hydrochloride combined with etomidate for painless induced abortion.Methods A total of 180 early pregnant women performed painless induced abortion admitted to the Department of Gynecology of the Third People's Hospital Affiliated to Fujian University of Chinese Medicine from May 2022 to May 2023 were prospective randpmly selected as the study objects and were divided into control group and observation group by random number table method,with 90 cases in each group.The control group was anesthetized with etomidate,and the observation group was anesthetized with hydromorphone hydrochloride on the basis of the control group.Heart rate and mean arterial pressure,postoperative contraction pain score,recovery time and adverse reaction were compared between the two groups.Results The postoperative contraction pain score was(0.80±0.31)points in the observation group,lower than(1.43±0.57)points in the control group,and the difference was statistically significant(t=9.211,P<0.001).The total incidence rate of adverse reaction in the observation group was 3.33%,which was lower than that in the control group(11.11%),the difference was statistically significant(χ^(2)=4.063,P=0.044).There were no statistically significant differences in mean arterial pressure,heart rate and recovery time between the two groups at different time(all P>0.05).Conclusion In painless induced abortion,hydromorphone hydrochloride combined with etomidate has better anesthetic effect,which can improve the contraction pain after operation and reduce the risk of anesthesia related adverse reaction.
