左归丸对核苷(酸)类药物抗病毒治疗致骨损伤的临床疗效观察

摘　　要：目的 观察左归丸对核苷(酸)类药物抗病毒治疗致骨损伤的临床疗效。方法 使用随机数字表法将2022年3月至2023年3月九江市中医医院长期使用核苷(酸)类药物抗病毒治疗致骨损伤的患者80例分对照组(n=40)和治疗组(n=40)。对照组予常规治疗,治疗组在对照组基础上给予中药左归丸口服,疗程均为12周。对比两组的临床疗效、肾功能、骨密度、血清骨代谢指标。结果 治疗组的有效率(87.50%)高于对照组(67.50%)(P<0.05)。治疗前两组中医症候积分比较,差异无统计学意义(P>0.05),治疗后治疗组的中医症候积分低于对照组(P<0.05);治疗前两组血肌酐(Cr)、尿素氮(BUN)、肾小球滤过率(eGFR)比较,差异无统计学意义(P>0.05),治疗后治疗组的Cr、BUN低于对照组,eGFR高于对照组(P<0.05);治疗前两组股骨颈骨密度、Z评分和T评分比较,差异无统计学意义(P>0.05),治疗后治疗组的股骨颈骨密度高于对照组,Z评分和T评分均低于对照组(P<0.05);治疗前两组血钙、血磷、碱性磷酸酶(ALP)比较,差异无统计学意义(P>0.05),治疗后两组的血钙、血磷比较,差异无统计学意义(P>0.05),治疗组的ALP高于对照组(P<0.05)。结论 左归丸对核苷(酸)类药物抗病毒治疗致骨损伤的临床疗效显著,能够改善患者的肾功能,提高骨密度,维持正常的血清骨代谢指标。Objective To observe the clinical effect of zuogui pills on nucleoside(acid)antiviral treatment of bone injury.Methods A total of 80 patients with bone injury caused by long-term antiviral treatment with nucleoside(acid)drugs in Jiujiang Traditional Chinese Medicine Hospital from March 2022 to March 2023 were divided into control group(n=40)and treatment group(n=40)using random number table method.The control group was given conventional treatment,and the treatment group was given zuogui pills orally.The course of treatment was 12 weeks.The clinical efficacy,liver and kidney function,bone mineral density and serum bone metabolism indexes were compared between the two groups.Results The effective rate of treatment group(87.50%)was higher than that of control group(67.50%)(P<0.05).Before treatment,the scores of TCM symptoms of the two groups were compared(P>0.05);after treatment,the scores of TCM symptoms of the treatment group were lower than those of the control group(P<0.05).Serum creatinine(Cr),urea nitrogen(BUN)and glomerular filtration rate(eGFR)of the two groups were compared before treatment(P>0.05);after treatment,Cr and BUN of the treatment group were lower than that of the control group,eGFR was higher than that of the control group(P<0.05).Before treatment,bone mineral density of femoral neck,Z score and T score were compared between the two groups(P>0.05);after treatment,bone mineral density of femoral neck in the treatment group was higher than that in the control group,and Z score and T score were lower than that in the control group(P<0.05).The serum calcium,serum phosphorus and alkaline phosphatase(ALP)of the two groups were compared before treatment(P>0.05),and the serum calcium and blood phosphorus of the two groups were compared after treatment(P>0.05).The ALP of the treatment group was higher than that of the control group(P<0.05).Conclusion Zuogui pills has significant clinical effect on nucleoside(acid)antiviral treatment of bone injury,which can improve patients'kidney function,increase bone
