不同频率重复经颅磁刺激联合头针治疗脑卒中后认知功能障碍临床研究

摘　　要：目的:观察不同频率重复经颅磁刺激联合头针治疗脑卒中后认知功能障碍的效果。方法:76例分为3组,对照组(26例)用头针治疗,低频治疗组(25例)用头针联合低频重复经颅磁刺激治疗,高频治疗组(25例)用头针联合高频重复经颅磁刺激治疗。结果:相较于对照组,高频治疗组和低频治疗组总有效率更高(P<0.05)。相较于治疗前,3组治疗后MMSE、ADL评分均提高,且治疗组均高于对照组(P<0.05),高频治疗组评分增加幅度更显著。相较于治疗前,3组治疗后NSE、S100β指标均降低,且治疗组低于对照组(P<0.05),高频治疗组指标下降幅度更显著。结论:高频和低频重复经颅磁刺激联合头针治疗脑卒中后认知功能障碍均有较好效果,高频治疗改善神经功能缺损,恢复智能效果更好。Objective:To observe the effect of repetitive transcranial magnetic stimulation at different frequencies combined with scalp acupuncture on cognitive dysfunction after stroke.Methods:Seventy-six cases were divided into 3 groups.The control group(26 cases)were treated with scalp acupuncture,the low-frequency treatment group(25 cases)were treated with scalp acupuncture combined with low-frequency repetitive transcranial magnetic stimulation,and the high-frequency treatment group(25 cases)was treated with scalp acupuncture combined with high-frequency repetitive transcranial magnetic stimulation.Results:Compared with the control group,the total effective rate of the high frequency treatment group and the low frequency treatment group was higher(P<0.05).Compared with those before treatment,the scores of MMSE and ADL in the three groups increased after treatment,and those in the treatment group were higher than the control group(P<0.05).The scores in the high-frequency treatment group increased more significantly.Compared with those before treatment,the NSE and S100βindexes of the three groups decreased after treatment,and those in the treatment group was lower than the control group(P<0.05),and the indexes of the high-frequency treatment group decreased more significantly.Conclusion:Both high-frequency and low-frequency repetitive transcranial magnetic stimulation combined with scalp acupuncture have good effects on post-stroke cognitive dysfunction.High-frequency treatment improves neurological deficits and restores intelligence better.
