摘 要:目的了解四川省8~10岁儿童甲状腺和碘营养现况并进行评价,为调整碘营养策略提供依据。方法2022年在四川省各市(州)选取1个县(市、区),在选取县的东、西、南、北、中5个方位各随机抽取1个乡(镇、街道),在所抽取的乡(镇、街道)随机抽取1所小学校并对该校8~10岁儿童40人,采用超声法检测甲状腺容积和结节,采集其尿样和家庭食用盐样各1份,检测尿碘和盐碘含量。采用SPSS 19.0软件进行描述性分析,率的比较采用χ^(2)检验,尿碘中位数比较采用Kruskal-Wallis H检验,尿碘中位数与盐碘均数相关性采用Spearman秩相关分析,检验水准α=0.05(双侧)。结果共调查8~10岁儿童4211人,甲状腺肿大率1.9%,甲状腺结节检出率0.6%;尿碘中位数192.6μg/L,尿碘<50μg/L的比例2.8%;碘盐覆盖率99.1%,碘盐合格率97.1%,合格碘盐食用率96.2%,加碘盐盐碘均数27.1 mg/kg、变异系数13.7%;尿碘中位数与盐碘均数存在弱相关(r_(s)=0.170,P=0.463)。结论四川省8~10岁儿童碘营养现况总体适宜,甲状腺肿等指标均达到碘缺乏病消除标准,在碘盐普及的情况下需进一步调查评价碘营养状况的影响因素。Objective To provide evidences for adjusting iodine nutrition strategies by understanding and evaluating the current status of thyroid and iodine nutrition on the 8-10 years old children in Sichuan province.Methods One counties(county-level cities or districts)was selected in each of cities(prefectures)of Sichuan province in 2022.With simple random sampling,one town(subdistrict)was selected respectively from each of the five divided directions in the east,west,south,north,and middle of each county,one primary school was selected respectively from each town(subdistrict),respectively.40 children aged 8-10 years were randomly selected from each primary school for checking and measuring the thyroid volume and nodule by using ultrasonic method,in addition the iodine content testing of their one urine sample and one edible salt sample at their home.SPSS 19.0 was used for descriptive analysis,and the comparison of rates was conducted by using Chi-square test,Kruskal Wallis H test was used to compare the median urine iodine,Spearman rank correlation analysis was used to examine the correlation between the median urine iodine and the mean salt iodine,α=0.05(bilateral).Results A total of 4211 children aged 8-10years were surveyed,the rate of goiter was 1.9%and the detection rate of thyroid nodule was0.6%,the median of urinary iodine was 192.6μg/L and the urine samples proportion of urinary iodine below 50μg/L was 2.8%.The iodized salt coverage rate,the qualified rate of iodized salt and the consumption rate of qualified iodized salt were 99.1%,97.1%and 96.2%,respectively.The iodine mean of iodized salt and coefficient of variation of iodine in iodized salt were 27.1 mg/kg and 13.7%,respectively.There was a weak correlation between the median urine iodine of 8-10years old children and the iodine mean of their household edible salt(r_(s)=0.170,P=0.463).Conclusions The current status of iodine nutrition in children aged 8-10 years is generally suitable in Sichuan province,all related indicators such as the rate of goiter a
