摘　　要：目的分析真实世界中高血压患者服用复方利血平氨苯蝶啶片控制血压的效果。方法回顾性选取2012—2019年青岛市西海岸新区就诊服用复方利血平氨苯蝶啶片的高血压患者,设为干预组;并按照性别、年龄和BMI 1∶1匹配未服用该药但服用了其他降压药物的高血压患者,作为对照组。从电子病例数据库收集患者的一般资料、用药和血压控制情况,采用线性、logistic和Cox回归分析服用该药与45 d血压控制和心血管疾病新发事件的相关性。结果共纳入4860例(干预组2627例,对照组2233例)患者,其中,3335例(68.6%)随访期内血压达标,360例(7.4%)发生心血管疾病新发事件。多因素线性分析和logistic回归分析显示,服用复方利血平氨苯蝶啶片会降低随访期末次收缩压(P<0.05),提高血压达标率(P<0.01)。多因素Cox回归分析显示,服用复方利血平氨苯蝶啶片是预防心血管疾病新发事件的保护因素,心血管疾病新发事件风险可降低20%(HR=0.80,95%CI:0.65~0.99);规律服用该药能进一步降低心血管疾病新发事件风险,降低达34%(HR=0.66,95%CI:0.45~0.97)。结论服用复方利血平氨苯蝶啶片可有效降低血压,降低心血管疾病新发事件发生风险;规律服用复方利血平氨苯蝶啶片的降压效果和心血管保护作用更为显著。Objective To analyze the effect of compound reserpine and triamterene tablets on blood pressure control among hypertensive patients in the real world.Methods Conducted a retrospective analysis based on Electronic Medical Records in Qingdao west Coast District from 2012 to 2019.Hypertensive patients who took compound reserpine and amitriptyline tab⁃lets were selected as the intervention group;and hypertensive patients who did not use the drug but used other antihypertensive medications were matched according to gender,age,and BMI in a 1∶1 ratio and regarded as the control group.General infor⁃mation,medication,and blood pressure control status of patients were collected.Linear,logistic,and cox regression analyses were comprehensively used to analyze the correlation between taking the drug and blood pressure control at 45 days and the incidence of new cardiovascular events.Results A total of 4860 patients were included(2627 in the intervention group and 2233 in the control group).Among them,3335(68.6%)achieved blood pressure targets during the follow-up period,and 360(7.4%)had new cardiovascular events.Multivariate linear and logistic regression analysis showed that taking compound reserpine and amphotericin tablets reduced the systolic blood pressure(P<0.05)and increased blood pressure compliance rate(P<0.01)at the end of the follow-up period.Multivariate Cox regression analysis showed that taking compound reserpine and amitriptyline tablets is an independent protective factor for cardiovascular new events,and the risk of cardiovascular new events can be reduced by 20%(HR=0.80,95%CI:0.65~0.99);regular use of this drug can lower the risk of new cardiovascu⁃lar events by 34%(HR=0.66,95%CI:0.45~0.97).Conclusion Taking compound reserpine and amphotericin tablets can effectively help control blood pressure and reduce the risk of new cardiovascular events;regular use of compound reserpine and amphotericin tablets shows more significant effects in lowering blood pressure and cardiovascular protection.
