摘 要:目的:评估MAKO机器人辅助膝关节内侧单髁置换术(UKA)的中期疗效。方法:回顾性分析2016年1月至2018年12月行MAKO机器人辅助UKA患者的临床资料,共28例33膝。其中男6例,女22例,年龄51~74岁,平均(62.6±7.5)岁,体重指数(BMI)为19.5~32.0 kg/m^(2),平均(26.2±3.3)kg/m^(2)。随访过程中评价影像学结果、膝关节功能,并观察并发症发生情况。影像学结果纳入假体在位率及假体角度偏差值。膝关节功能评价包括美国膝关节协会评分(KSS)、牛津大学膝关节评分(OKS)、患者满意度及关节遗忘评分(FJS)。结果:所有患者均获得随访,随访时间44~59个月,平均(51.1±6.0)个月。末次随访33膝单髁假体均在位良好,且与术后1年相比未出现假体角度的异常偏差值。末次随访时患者KSS及OKS均较术前改善,也均优于术后1年,但差异无统计学意义(P均>0.05);末次随访时患者满意度评分为(35.0±1.9)分,FJS为(89.2±2.9)分。3例患者发生并发症,其中2例为膝前痛,1例为膝关节外侧疼痛。结论:MAKO辅助膝关节内侧UKA中期随访的假体在位率好,未出现异常的假体位置,膝关节功能及患者报告临床预后结果(PROMs)优秀,获得了较好的临床疗效。Objective:To evaluate the medium-term clinical outcomes of MAKO robot-assisted medial unicompartmental knee arthroplasty(UKA).Methods:A retrospective analysis was performed on patients underwent MAKO robot-assisted UKA from January 2016 to December 2018.Totally 28 patients with the mean age of(62.6±7.5)years(range,51-74 years)were included and there were 6 males and 22 females.The mean score of the body mass index(BMI)was(26.2±3.3)kg/m^(2)(range,19.5-32.0 kg/m^(2)).The imaging finding and joint function were evaluated during the follow up,and the complications were observed.The rate of in-place prosthesis and angular deviation of the prosthesis were analyzed with imaging.The evaluation of joint function included Oxford knee score(OKS),knee society score(KSS),the satisfaction of the patients and forgotten joint score(FJS).Results:All patients were followed up with a mean duration of(51.1±6.0)months(range,44-59 months).All prosthesis in 33 knees were alive and in place,and no abnormal angular deviation of the prosthesis was observed compared with that one year after the operation.KSS and OKS were significantly improved compared with pre-operation and one year after the operation with no significant difference(P>0.05)at the final follow-up.The mean satisfaction score was(35.0±1.9)points,and the mean FJS was(89.2±2.9)points at the final follow-up.Three patients reported complications including 2 patients with anterior knee pain and one patient with lateral knee pain.Conclusions:In medium-term followup of MAKO robot-assisted UKA,the prosthesis orientation outliers are not observed.Joint function and patient reported outcome measurements are excellent.MAKO robot-assisted UKA achieves satisfied clinical outcomes.
