36例犬椎间盘突出病例的磁共振诊断和治疗的回顾性分析  

36 Cases of Intervertebral Disc Herniation in Dogs: a Retrospective Analysis of Diagnosis and Treatment with Magnetic Resonance Imaging

摘　　要：犬椎间盘突出是临床常见疾病,许多病例由于发病初期没有及时确诊并采取对症治疗,导致病情难以控制,错失最佳治疗时机。本文对2020年1月—2021年1月在北京派仕佳徳动物医院就诊的36例犬椎间盘突出病例的基本情况、诊断和治疗进行回顾性分析。结果显示,36例病例中雌性5例,雄性31例;1~7岁犬24例,7岁以上犬12例;占比最高的3种品种分别是法国斗牛犬(41.67%,15/36)、泰迪犬(13.89%,5/36)和田园犬(11.11%,4/36)。通过神经学检查和磁共振成像(MRI)诊断,所有患犬通过MRI检查确诊为Hanson I型椎间盘突出;脊髓发病部位中胸腰节段(T3-L3)占比最高(50.00%,18/36),其次为颈椎前段(C1-C5)(38.89%,14/36);按照神经损伤程度分级,四级占比最多(44.44%,16/36),其次为三级(36.11%,13/36)。16例患犬病例经颈椎腹侧开槽术治疗后全部康复,20例患犬病例经胸腰椎半椎板切除术治疗后19例康复,手术成功率为100%,术后康复率为97%。本研究为犬椎间盘突出疾病的诊断和治疗,以及磁共振成像技术的临床应用提供重要参考价值。Intervertebral disc herniation is a common clinical condition in dogs.Many cases,due to delayed diagnosis and treatment at the onset of the disease,result in difficult-to-control conditions and miss the optimal treatment window.This study retrospectively analyzed the basic information,diagnosis,and treatment of 36 cases of canine intervertebral disc herniation treated at the Beijing Petsguard Animal Hospital from January 2020 to January 2021.The results showed that among the 36 cases,there were 5 females and 31 males;24 dogs were aged between 1 to 7 years,and 12 dogs were aged 7 years and above.The three most common breeds were French Bulldog(41.67%,15/36),Poodle(13.89%,5/36),and Mongrel(11.11%,4/36).Through neurological examination and magnetic resonance imaging(MRI)technology diagnosis,all affected dogs were diagnosed with Hansen Type I disc herniation confirmed by MRI examination.The thoracolumbar region(T3-L3)accounted for the highest proportion of spinal cord lesions(50.00%,18/36),followed by the cervical anterior segment(C1-C5)(38.89%,14/36).According to the degree of neurological injury,Grade IV accounted for the highest proportion(44.44%,16/36),followed by Grade III(36.11%,13/36).Among the 16 cases treated with ventral slot decompression,all recovered completely,and among the 20 cases treated with hemilaminectomy,19 recovered,resulting in a surgical success rate of 100%and a postoperative recovery rate of 97%.This study provides important reference value for the diagnosis and treatment of intervertebral disc herniation in dogs and the clinical application of magnetic resonance imaging technology.
