猫肝脏淀粉样变引起自发性间歇性血腹的病例分析

摘　　要：猫肝脏淀粉样变是一种会引起肝脏自发性破裂出血的疾病,发病机制和发病原因尚不清楚。本院接诊了2例自发性间歇性血腹并发慢性肾病的患猫病例,在外院均被诊断为传染性腹膜炎。转诊至本院后,2只患猫经病史调查、体格检查、实验室检查、影像学检查和肝脏组织病理学检查,最终被诊断为肝脏淀粉样变。2只患猫确诊后经支持治疗病情获得短暂缓解,但慢性肾病在数次自发性血腹发作后逐渐加重,并于确诊后半年内死亡。本文对2例病例的诊疗思路进行了总结和讨论,以期为此类疾病的临床诊疗提供参考。Hepatic amyloidosis in cats is a disease that can lead to spontaneous liver rupture and hemorrhage,and the pathogenesis and etiology are not well understood.Two cases of spontaneous intermittent hemoabdomen complicated with chronic kidney disease in cats were admitted to our hospital,and both were diagnosed with infectious peritonitis at previous clinics.After referral to our hospital,the two cats underwent thorough medical history investigation,physical examination,laboratory tests,imageological examination,and liver histopathological examination,leading to a diagnosis of hepatic amyloidosis.Following supportive treatment,the condition of the two affected cats improved temporarily;however,chronic kidney disease worsened progressively after several episodes of spontaneous hemoabdomen,and both cats succumbed within six months of diagnosis.This paper summarizes and discusses the diagnostic and therapeutic approaches for these two cases,aiming to provide insights into the clinical management of such diseases.
