内蒙古自治区赤峰市妊娠期妇女血清特异性甲状腺功能指标的检测及分析

摘　　要：目的探讨赤峰地区健康孕妇不同妊娠时期血清特异性促甲状腺激素(TSH)和游离甲状腺素(FT4)的参考范围,为本地区妊娠期妇女的甲状腺疾病提供诊断依据。方法统计2018年1月1日至2022年12月31日就诊于赤峰市妇产医院和松山区妇幼保健院的2208例赤峰地区健康孕妇的甲状腺功能检测值,按照孕妇检测日期分为孕早期(0~13周)、孕中期(14~27周)、孕晚期(28~40周),统计不同时期孕妇血清TSH和FT4水平,以P 2.5作为参考值的下限,P 97.5作为参考值的上限,确定特异性TSH和FT4的参考范围。结果纳入的研究对象年龄在21~42岁。统计分析显示,本地区TSH的孕早期参考范围为(0.16~4.21)U/L,孕中期的参考范围为(0.40~4.02)U/L,孕晚期的参考范围为(0.48~4.47)U/L;FT4的孕早期参考范围为(8.11~14.54)pmol/L,孕中期的参考范围是(6.82~12.74)pmol/L,孕晚期的参考范围是(7.08~14.41)pmol/L。本研究结果与参考的贝克曼说明书参考范围及以往报道有一定差别。结论建立赤峰地区不同妊娠期特异性TSH和FT4正常参考范围,利于妊娠期妇女甲状腺疾病的准确诊断和治疗,也可为本地区降低出生缺陷和优生优育提供数据支持。Objective:To explore the reference range of serum specific thyroid stimulating hormone(TSH)and free thyroxine(FT 4)of different pregnancy periods among healthy pregnant women in Chifeng in order to provide diagnostic basis for thyroid diseases in pregnant women of the target area.Methods:The values of thyroid function test among 2208 healthy pregnant women in Chifeng who were treated in Chifeng Maternity Hospital and Songshan Maternal and Child Health Hospital from 2018 to 2022 were analyzed and the gestation period was divided into the first trimester(0~13 weeks),the second trimester(14~27 weeks)and the third trimester(28~40 weeks)according to the test date.The 2.5th percentile(P 2.5)was used as the lower limit of reference value and 97.5th percentile(P 97.5)as the upper limit of the reference value to determine the reference range of specific TSH and FT 4.Results:The age of the women was between 21 and 42 years old.The reference range of TSH was(0.16~4.21)U/L in the first trimester,(0.40~4.02)U/L in the second trimester and(0.48~4.47)U/L in the third trimester.The reference range of FT 4 was(8.11~14.54)pmol/L in the first trimester,(6.82~12.74)pmol/L in the second trimester,and(7.08~14.41)pmol/L in the third trimester.The results of this study differed to some extent from the reference range of the Beckman manual used in the past and other reports.Conclusions:The establishment of the normal reference range of TSH and FT 4 in different gestation period in Chifeng would benefit for preciously diagnosis and treatment of thyroid diseases and provide data support for reducing birth defects and eugenic in the target area.
