糖化血红蛋白长期变异性与2型糖尿病患者心功能指标和左心室质量指数相关性的研究

摘　　要：目的 探讨HbA_(1)c长期变异性与T2DM患者心功能指标及左心室质量指数(LVMI)的相关性。方法 选取2019年7月至2020年7月海南医学院第二附属医院全科医疗科住院治疗的T2DM患者158例,随访2年后12例失访,最终纳入146例,按照LVMI中位数分为低LVMI组和高LVMI组,每组各73例。Pearson相关分析HbA1c标准差(HbA1c-SD)与Δ左心室射血分数(ΔLVEF)、Δ二尖瓣舒张早期血流速度峰值/二尖瓣环舒张早期运动速度(ΔE/e’)和LVMI的相关性,多元线性回归分析T2DM患者LVMI的影响因素。结果 高LVMI组年龄、DM病程、ΔBMI、FPG-mean、HbA1c-mean、HbA1c-SD、ΔLVEF高于低LVMI组(P<0.05),ΔE/e’低于低LVMI组(P<0.05)。Pearson相关分析显示,HbA1c-SD与ΔE/e’、LVMI呈正相关(P<0.05),与ΔLVEF呈负相关(P<0.05)。多元线性回归分析显示,年龄、DM病程、ΔBMI、HbA1c-SD是T2DM患者LVMI的影响因素。结论 T2DM患者HbA1c长期变异性与心功能及LVMI相关。Objective To explore the relation between long term glycosylated hemoglobin Alc(HbAic)variability and indicators of cardiac function as well as left ventricular mass index(LVMI)in type 2 diabetes mellitus(T2DM)patients.Methods 158 T2DM patients hospitalized in the Second Affiliated Hospital of Hainan Medical College from July 2019 to July 2020 were recruited.After two years of follow-up,12 cases were lost and 146 cases were ultimately included and divided into low LVMI group(n=73)and high LVMI group(n=73).The relation between HbAic-SD and△left ventricular ejection fraction(△LVEF),△ratio of mitral peak velocity of early filling to early diastolic mitral annular velocity(△E/e')and LVMI were analyzed by pearson correlation analysis.The multiple linear regression were performed to analyze the related factors of LVMI in T2DM.Results Age,disease duration,△body mass index(BMI),FPG-mean,HbAic-mean,HbA,c-SD and HbAic-mean,HbAic-SD and△LVMI in high LVMI group were higher than those in low LVMI group(P<0.05),and△E/e'was lower than that in the low LVMI group(P<0.05).Pearson correlation analysis showed that there was a positive correlation between HbA,c-SD and△E/e'or LVMI(P<0.05),but a negative correlation between HbAic-SD and△LVEF(P<0.05).The multiple linear regression showed that age,disease duration,△BMI and HbAic-SD were the related factors of LVMI.Conclusion Long term HbAic variability is related to cardiac function and LVMI in T2DM patients.
